| ObjectiveBreast cancer has the highest incidence and mortality among women worldwide, and the incidence is increasing. However,in recent years, there have been no reports on the clinical and pathological features of breast cancer patients in Shandong Province. In this study,we retrospectively analyzed the clinicopathological data of breast cancer patients who underwent surgery in Shandong Provincial Hospital from July 2010 to June 2018 to investigate the clinical and pathological characteristics of breast cancer patients in Shandong Province in recent years and their annual trends.MethodsThe clinical and pathological data of patients with new-onset breast cancer who visited the Department of Breast and Thyroid Surgery,Shandong Provincial Hospital from July 2010 to June 2018 and underwent surgery were retrospectively analyzed, including age, gender, tumor size,multicentricity/multicentricity,bilateral breast cancer, pathological type, lymph node metastasis, vascular, neurological, skeletal muscle and skin invasion,TNM stage, histological grade, molecular classification and breast concomitant diseases. The information was derived from the medical record of the cases, and SPSS 22.0 software was used for statistical analysis. The clinical and pathological characteristics were described and analyzed by relative number and constituent ratio, and the annual changes of the rate were analyzed by curve regression analysis.ResultsA total of 3514 patients with breast cancer were included during the past 9 years. The gender(male/female)ratio was 1.00:318.45,The average age of onset was 51 years. The median age of onset was 50 years. The peak age of onset was 40~50 years. The highest peak age was 45~50 years. Both the average age and median age of onset of breast cancer increased during the past 9 years(P=0.021,P=0.024).Synchronous bilateral breast cancer accounted for 1.05%of all breast cancer patients, and multicentric/multicentric breast cancer accounted for 5.07%of all breast cancer patients. The main pathological type was invasive ductal carcinoma, followed by intraductal carcinoma. There was a significant difference in the constituent ratio of pathological types of breast cancer patients during the 9 years(P<0.001).There was a significant difference in the annual constituent ratio of patients with TNM stage 0 and stage I breast cancer(P=0.004,P=0.009).There was a significant difference in the annual constituent ratio of histological grade Ⅱ,Ⅱ~Ⅲ,and Ⅲ(P<0.001).There were significant differences in the annual constituent ratios of LuminalA,LuminalB and HER-2 positive(HR negative)types(P<0.001,P<0.001,P=0.032).The difference in the constituent ratio of breast cancer accompanied by hyperplasia of mammary glands during 9 years was statistically significant(P=0.047).The difference in the proportion of vascular invasion by the tumor during the 9 years was statistically significant(P=0.018).There was also a significant difference in lymph node metastasis among breast cancer patients during the 9 years(P=0.003).The proportion of patients with synchronous bilateral breast cancer increased during 9 years(P=0.020),and there was no significant increase in the proportion of patients with multicentric/multicentric breast cancer. There was no significant trend in pathological type and molecular classification. The proportion of patients with stage I breast cancer increased during 9 years(P=0.025),and the proportion of patients with stage IIIC breast cancer showed a decreasing trend(P=0.028).Tumor invasion of the skin showed a decreasing trend during 9 years(P=0.031).Lymph node metastasis decreased during 9 years(P=0.038).ConclusionThis study elucidated the age of onset, gender ratio, proportion of synchronous bilateral breast cancer and multicentric/multicentric breast cancer, pathological type, molecular classification,TNM stage, lymph node metastasis, concomitant breast diseases and other clinical and pathological characteristics of 3514 patients with breast cancer and the trends of these characteristics during the 9 years from 2010 to 2018,which can provide more information for the study of clinicopathological characteristics of breast cancer patients in Shandong Province. |
