| Objective: In recent years,the age of breast cancer tends to be younger and the prognosis of young patients with breast cancer is poor.At present,there is no standard treatment for young breast cancer in the clinic.Therefore,the case data of young women with breast cancer are collected,and the factors affecting prognosis are analyzed.Furthermore,result of analysis will provide the reference for individualized treatment and prognosis of breast cancer in young women.Methods: The clinical data of patients with primary breast cancer from stage I to III who were admitted to the Hebei Cancer Hospital from January 2015 to December 2015 were selected.According to the 2012 European Association of Breast Cancer Professionals,Age of young breast cancer was defined less than 35 years old,88 patients were divided into young group(<35 years old,47 cases)and old-age group(≥35 years old,41cases).the data of pathological features and prognosis between the two groups was analyzed.Results:1.Compared with the pathological features of the old-age group,the young group tend to have negative expression of estrogen receptor(ER)(P = 0.002)and positive expression of human epidermal growth factor receptor 2(HER2)(P = 0.005).There was no significant statistically difference in lymph node metastasis,tumor size,progesterone receptor(PR)expression,clinical stage,and adjuvant therapy in two groups(P≥0.05).2.In the survival analysis,the 3-year disease-free survival(DFS)difference between the young group and the old group was significant statistically(P=0.001),and the 3-year overall survival(OS)difference was not significant statistically(P=0.177).There was a statistically significant difference in DFS between the young group and the old-age group in the Ⅱ and Ⅲ stage patients,but there was no significant difference between the two groups in the Ⅰ stage.3.In the multivariate prognostic analysis of stage II and III patients,clinical stage II,III and age less than 35,positive expression of HER2 are independent adverse prognostic factors,which influence 3 years DFS(P <0.05).The factors of tumor maximum diameter>2cm,negative expression of ER,negative expression of PR,and lymph node metastasis are not related to DFS and OS.(P>0.05).Conclusion:1.Young women with breast cancer have the characteristics of high ER negative expression and high positive HER2 expression.2.The prognosis of young women with breast cancer is poor,and which significant among stage II and III breast cancer patients.3.Poor prognosis may be related to age<35,clinical stage II and III,and HER2 positive expression in stage II and III patients. |
