Objective1. To study the incidence and duration of residual dizziness after successful repositioning treatment in patients with benign paroxysmal positional vertigo as well as the clinical factors associated with the residual dizziness.2. To study the mental state of the patients with/without residual dizziness.Methods1.284cases of BPPV patients were registered outpatient including the clinical manifestations, duration of vertigo, total number of vertigo attacks, the number of repositioning maneuvers, history of BPPV, etc. find the incidence and duration of the dizziness through the weekly face to face, telephone follow-up. Related statistical methods were used to identify for the clinical factors.2. According to the residual dizziness complained after successful repositioning treatment in patients with BPPV, they were classified as group A1(with dizziness) or group B (without dizziness). Based on residual symptoms reported during three follow-ups, group A2、A3、B2、B3were further divided respectively in14-15d,29-30d after successful repositioning treatment and the Hospital Anxiety and Depression Scale (HADS), anxiety (SAS) and depression (SDS), self-rating scale were completed. And the normal control group (C) was set at the same time. Use analysis of variance to compare the differences and similarities of the scores among groups; Age and gender composition of the ratio were compared.Results1. There were284patients with confirmed BPPV,245cases complete the follow-up of3months finally. And158(55.63%) of them complained of residual dizziness after successful repositioning treatment, Of the158patients,141cases underwent the follow-up of3month, there are still19/245(7.76%) cases complained dizziness at the90th d. The registered data show that the average age of the group A1(with dizziness) is significantly higher than group B1(without dizziness)(61.46±9.38vs56.93±7.62,P=0.015). In addition, the incidence of residual dizziness of female is higher than the male (54.69%vs45.31%,)(P=0.005). Logistic regression analysis show that the duration of vertigo before treatment is the independent risk factor for dizziness after BPPV.2.208patients completed the psychological scale tests after successful repositioning treatment. There are no significant difference on HADS, SAS, SDS scores、gender and age among group A1group B1and group C during the first follow-up. The HADS anxiety score ((10.50±4.87) and depression score (7.41±3.73) of the patients with the second follow-up (13-15d after cured) still complain dizziness (group A2, n=61) was significantly higher than the first follow-up with dizziness while dizziness disappeared this time (group B2> n=34)(P<0.001). There is no significant difference between group B1(n=113) and group B2(n=34) on anxiety, depression scores (P>0.05) with the Second follow-up. With the similar results on the SAS, SDS scores during the second follow-up. Patients (A3, n=24) during the third follow-up (29-30d), their HAD anxiety (14.20±3.70) and depression scores (11.20±3.77) were significantly higher than the group B2, B1and C. What Interesting is the scores of anxiety (7.50±4.37) and depression (7.16±4.67) scales of the first and second follow-ups with dizziness complained while the third follow-up disappeared (group B3,n=37) was significantly higher than the group B2(n=34)(P<0.001).Conclusion1. More than half of the patients included in this study complained of residual dizziness after successful repositioning. And its disabling effects are more noticeable in females and older patients.2. Duration of the vertigo before treatment is the independent risk factor for residual dizziness.3. Patients with dizziness last more than two weeks are more associated with anxiety or depression.
