Effect Factors Of Benign Paroxysmal Positional Vertigo Prognosis Analysis

Object: Benign paroxysmal positional vertigo(BPPV)is the most common cause of vertigo.Despite most of the patients can completely recover from the symptom through standard therapy,the recurrence of BPPV is not rare,besides,some patient can catch persistent postural perceptive dizziness(PPPD)secondary to BPPV.Methods: The medical records of the patients firstly diagnosed with BPPV in Neurology Clinic,Chongqing Medical University Affiliated Second Hospital from July 2017 to June 2016,whose age is between 18 to 79 years old were collected consecutively and concretely.After 6 months from onset,telephone follow up was executed to measure sleep quality through Pittsburgh Sleep Quality Index and the condition of patients' psychological state through Hamilton's Anxiety Scale(HAMAs)and Hamilton's Depression Scale(HAMDs).And get full knowledge about BPPV recurrence and if PPPD secondary to BPPV happens.Result: Total 158 cases were collected,among which in the first 6 month,the recurrence was 21.52% and the rate of PPPD secondary to BPPV was 18.35%.Between the recurrent group and non-recurrent group,secondary PPPD and non-secondary PPPD group,PSQI?HAMAs and HAMDs score showed statistic difference.Logistic regression analysis indicated that PSQI was an independent factor to BPPV recurrence,while HAMDs score was an independent factor to the rate of onset of PPPD secondary to BPPV.Besides that,Alcohol intake also showed statistic difference between secondary PPPD and non-secondary PPPD group.Conclusion: Sleep quality,anxiety,depression quality cooperatively affect the prognosis of BPPV about both the recurrence and the rate of onset of secondary PPPD independently.Sleep quality affects BPPV recurrence independently,while depression have independent effect on the occurrence of PPPD secondary to BPPV.Alcohol intake is one of the risk factors in PPPD secondary to BPPV.
