| Objective:Through investigating and analysising community care needs of stroke patients in NanChang city, the study could understand the situation on community care needs of stroke patients. To develop stroke community nurse,research results could be provided for government policy makers in our city.Meanwhile,the study might provide the reference for government to carry out the complete community stroke care services. The study goal was to meet community care needs of stroke patients, enhance the community nursing quality,and achieve the purpose of promoting patients recover.Methods:From April 2010 to February 2011, investigator took multistage sampling method to investigate 220 stroke patients in three administrative regions of JiangXi province NanChang city,and altogether collected effective investigation questionnaire 207 copies. Research tools used three questionnaires:community stroke patients personal material questionnaire, Barthel index evaluation table and community stroke patients care needs questionnaire. Community stroke patients personal material questionnaire and community stroke patients care needs questionnaire were filled out by patients.In common, Barthel index evaluation table were calculated total scores by investigators finally after patients filled. Bilding the database after data collection finished,investigator used SPSS 17.0 version to analysis data,and analysised reliability and validity of community stroke patients care needs questionnaire firstly. Through the experts evaluated, investigator made the survey tools of eight dimensions,50 entries.All dimensions Cronbach's alpha coefficients range were 0.75～0.93, repeated measure reliability index was 0.91 and content validity index was 0.89.Statistical methods:the patient's personal datas were taken descriptive analysis, two sample t-test, single factor analysis of variance, LSD inspection or S-N-K inspection.The datas of Barthel index evaluation table were taken the single factor analysis of variance. The factors affecting of community stroke patients care needs were taken multiple stepwise regression analysis.Results:1. For stroke patients in communities, health education care needs were highest (31.76±3.390), and social support needs were lowest (10.14±3.104)2. (1) Female stroke patients were higher than male patients,and no-spouse stroke patients were higher than married ones in life care needs, complications care needs, professional care needs and rehabilitation guidance needs.(2) Seventy patients needed lowest in increasing self-care capability (18.10±3.351). Other care needs of them were higher than under 70 patients.(3) Life care needs were highest in primary grades stroke patients(15.95±3.354),and undergraduate patients needed lowest in increasing self-care capability care needs(17.07±3.463).(4) Hemorrhage patients needed highest in health education care needs(32.30±3.215),and subarachnoid hemorrhage patients needed highest in increasing self-care capability care needs(22.00±2.268). (5) After stroking 1-6 months,patients needed lowest in life care needs(13.14±3.282). Above 3 years after stroking, patients needed highest in community nursing total needs(136.52±24.155). (6) More than three stroke times, patients needed highest in community nursing total needs(157.31±15.787).(7)Patients of ADL scoring 100 needed lowest in community nursing total needs(102.77±12.272).3. These factors had influenced community stroke patients care needs,which were ADL score, hemiplegia, aphasia, difficulty swallowing, cerebral infarction, economic income, still work not adjust working, businessman and heart disease. (R2= 0.777, F=76.481, P=0.000).Conclusion:1. The female and no-spouse stroke patients needed higher in life care needs and rehabilitation care needs.2. Older stroke patients few needed in increasing self-care capabilitythe care needs.And they had self-care defects.3. The high cultural degree stroke patients could master knowledge of disease well.And their self-care ability were best. 4. With Long course, suffering from high blood pressure and more than one time stroking, hemorrhage patients needed highest in health education care needs.With short course,little stroke time and no-hemiplegia, subarachnoid hemorrhage patients needed highest in increasing self-care capability care needs.5.Stroke patients with short course had better self-care ability. With long course and several stroke times, stroke patients needed higher in social support care needs and complications care needs.6. Stroke patients with ADL high score needed lowest in community total care needs.7. There were nine factors influencing in community care needs of stroke patients. Among which the biggest influence was ADL score, followed by hemiplegia and the minimum was heart disease. |
