The Observation Of Clinical Effect Of Gu Shang Fu Yuan Soup Promoting Ossification In The Callus Traction Region Of The Bone Defects

Objective:To explore the definite effect of gu shang fu yuan soup promoting ossification in the callus traction region of the bone defects,which can provide the theory foundation for clinical use.Methods:40 patients who had extensive bone defects on tibia and corresponded with the inclusion criteria were performed by retrospe- ctive study.20 cases were intervened by gu shang fu yuan soup and 20 cases were intervened by no medicines,which were set for gu shang fu yuan soup and controller respectively. One-stage debridement was carried out in the damaged limbs in all cases,repairing the damaged blood vessels and nerves. Surgical methods can be divided into three types. I categories:debridement fixed+tissue transplants+osteotomies+bone transport, suitable for GustiloⅢtype B trauma.Ⅱcategories:debridement fixed+vascular/nerve repair+ tissue transplants+osteotomies+bone transport, suitable for GustiloⅢC trauma.Ⅲcategories: debridement fixed+vascular/nerve repair+tissue transplants+Phase ii osteotomies, apply to the agreement by area there are no blood essels and adopt bridge type crossover vascular anastomosis dissociation tissue transplants. All cases in osteotomies postoperative 14 days were performed by bone handling at a rate of 0.25mm 4 times a day.The cases of gu shang fu yuan soup group began to take gu shang fu yuan soup 1 week postoperation,continuing 6 weeks.There was no drug intervention in the controller. Radiographic examination was peformed in the forth week, eighth weekand twelfth week after bone transport for all the subjects,measuring and calculating the CDR(callus diameter ratios). If there was no obvious callus or the contact surface was too small in the bone ends,positive bone graft should be performed at the bone ends.When the radiographic examination demonstrated that there was vague callus and cortex bone,and the CDR≥85%,the healing of bone determined,and the external fixation could be removed.Results:All the grafting tissue survived,and there was necrosis in the distal skin margin of flap in 3 cases,which healed after dressing change. Nail urethral infection happened in 4 cases,which healed after antibiotic therapy. All the bone stumps healed.There were 29 direct-healing cases.Bone graft was performed in 11 cases,becanse fibrous joint emerged in the bone far-end.There was no second factures and angulation deformity.The mean value of CDR in the gu shang fu yuan soup group in the forth week,eighth week and twelfth week were bigger than that of controller,and there was significant difference in statistics. There was significant difference between the amounts of bone graft cases in two groups. The mean value of the bone healing time of the gu shang fu yuan soup group was shorter than that of controller.And there was significant difference in statistics.Conclusion:Gu shang fu yuan soup used in the early and middle phases of bone transport could promote osteogenesis in the callus traction region.The treatment period was shorten.The gu shang fu yuan soup treatment had obvious superiority over the controller.
