Objective To summarize the clinical effect of Bifocal Approach with Orthofix external fixator in the treatment of the tibia defects.Methods Clinical data of 23 patients with tibial defects treated by bone transport with Orthofix external fixator in the Department of Micro-Reconstructive Surgery,First Affiliated Hospital of Xinjiang Medical University from January 2010 to December 2016 were retrospectively analyzed.There were 15 males and 8 females with a total of 23 cases.The age ranged from 14 to 66 years,with an average age of 40.0 years.Tibial bone defect was 3.5~13cm.According to the length,diameter and type,length of tibial bone defect to install the Orthofix external fixator and choose the style of the osteotomy.After the debridement,according to the length of the tibia implant the external fixator Gigli saw technique osteotomy.18 cases of bifocal osteotomy and 5 cases of trifocal osteotomy.At the rate of 1.0mm/d to transport the bone for bifocal osteotomy and 2.0 mm/d to transport the bone for trifocal osteotomy.The mineralization time,bone lengthening length,fixation time or union indexes,as well as complications were recorded during the bone transport.Results All of the 23 patients were followed up.The mean follow-up time was 20(12~24)months.The bone lengthening was 6.5±2.9cm,fixation time is 10.1±3.9M,union indexes was 48.0±5.1 d/cm.The clinical effect of fracture healing using Paley scoring standard.Conclusion Bone transport with Orthofix external fixator can repair the defects of tibia,and it should be given positive early prevention for the complications during bone transport.
