ObjectiveThe purpose of this study is to observe the clinical effect of the method of blood-activating and reunion of bone and bone transport technique on large bone defects,so as to provide a more feasible and effective treatment on patients with bone defects after severe trauma,infectious nonunion of bone or large bone defects after osteomyelitis debridement.Methods60 patients with large bone defects,who accepted a treatment in the department of microtrauma orthopedics of Zhongshan Traditional Chinese Medicine Hospital from November,2015 to September,2017 and were in accordance with the criteria of diagnosis,inclusion and exclusion,were selected and divided into the treatment group and the control group,30 cases in the treatment group and 30 in the control group,with the random number table.Both groups were treated with the basic treatment,including Ilizarov bone transport and external fixation,the combination of rest and function exercise,daily routine wound dressing,anti-inflammatory and analgesic treatment and so on.For the treatment group,Taohongsiwu decoction and Shangke Jiuwei Jiangu tablet were used before one week of the operation based on the basic treatment.And the control group,on the basis of the basic treatment,was treated with Taohongsiwu decoction before one week of the operation and the course of the treatment was 6months.And for each two weeks,information about bone mineral density(BMD,its unit is g/cm2),Player scores,healing time and healing index were observed and collected for statistical analysis.ResultsAll of the 55 patients in this study were treated with bone transportsurgery and a follow-up visit about 7 to 20 months,11 months averagely,was given to 55 cases except for 2 patients lost in the follow-up trip,2 patients of drug intolerance and 1 patient of refracture caused by accidental fall in the course of bone transport.(1)Before the second week after the operation,the BMD(g/cm2)of the patients in two groups were increased in different degrees,but the comparison of BMD of two groups was P=0.818>0.05,and there was no significant difference in statistics.After 4 weeks,the value was P=0.000<0.05,and there was remarkable difference in statistics.(2)The comparison of the data of Paley scores of two groups was P=0.039<0.05,and there was significant difference in statistics.(3)There was significant difference in healing time between the two groups(P=0.005 < 0.05);the healing indexes were 1.469±0.138 and 1.674±0.144 for the treatment group and the control group respectively,P=0.000<0.05,and there was significant difference in statistics.Conclusion:The method of blood-activating,combined with reunion of bone and Ilizarov bone transport technique,has special effects in the treatment of large bone defects with increases in the growth speed of bones during the bone transport,and reduces the time of bone transport and bone mineralization. |
