IntroductionVascular Dementia (VaD) is the main cause of dementia in Japan, China as well as many countries in Asia and recognized by WHO as a main cause of morbidity in the elderly.The clinical presentations of VaD can be separated into cognitive impairments (core symptoms) or non-cognitive impairments (sub-symptoms), like hallucination, delusion, anxiety and depression. The later is also known as Behavioral Psychological Symptoms of Dementia (BPSD). Previously, the focus of VaD is in its effect in impairment of cognition and intelligence. However, it is realized that BPSD is the main cause of morbidity; not only it impairs the ability of daily living, but also creates distress to the caregivers and society, as well as financial loss and dangers to the patient and others. GuanYuanKeli (KanGenKaryu, KAN) product of Iskra, Japan, contains 6 Chinese herbs, Salvia miltiorrhiza (Danshen), Carthamus tinctorius (Honghua), Lingusticum Chuanxiong (Chuanxiong), Paeoniae Radix (Shaoyao), Cyperus rotundus,L. (Xiangfu) and Saussurea lappa Clarke (Muxiang). It has been widely used clinically in treating cardio and cerebral vascular disease.ObjectiveThis study is conducted in 4 parts, one clinical study and three laboratory studies.Clinical study examined the effect, efficacy and safety of KAN in cognitive and BPSD in VaD patients using several psychological rating scale for dementia.Laboratory studies evaluate the effect of KAN in repeated cerebral ischemia rat model in spatial memory, effect in survival nerve count, and reduction of infarct area in 4VO model and MCAO model. The psychological effect of KAN is also studied using various induced behavioral impairment rat model.Materials and Methods1.In clinical study, 52 patients with VaD (following DSM-IV,NINDS-AIREN, HDRS criteria) are randomized into Treatment group (KAN), n=28, and Control group, n=26.
