Objective:To give some recommendations of establishing the specialist physician system in Tianjin, the methods of the specialist training, access and management system of domestic and overseas were collected and compared, the actual situation of standardized training of resident in Tianjin was investigated, the major human resource management factors that could impact the establishment of specialist physician system in Tianjin were analyzed, including the personal arrangements, management organization, training cost, the compensation designing and management, rewards and disciplinary on specialist, and so on.Method:Document study:collect and search the content and methods of specialist training, access and management system in other countries or regions, with the relevant information we did the systematic analysis and comparison, from the Ministry of Health Information Network to get the specialist physician system related policies, laws, regulations, acts and comprehensive measures, as well as the situations in some provinces and municipalities which have already carried out this system.â‘¡Sampling survey:1100 managers, clinical experts, residents and medical education managers in 18 secondary and tertiary hospitals in Tianjin were selected using cluster and stratified sampling. Meanwhile, the collected information were described and analyzed.â‘¢Experts consult:the experts in the fields of health management, health economics or macro-economic, sociology, public health were consulted, and discussed the specialist physician system deficiencies and problems in Tianjin, which all based on the analysis of the theme of the research and the questionnaire results.Results:93.8%of the investigates thought the establishing of Accreditation Council for Graduate Medical Education is necessary, and it's duty is responsible for the implementation of medical specialist training, and there was no statistic differences among the different groups of people (P> 0.05); and there were 74.9% thought that the training cost should be shared by the government and the individual, 84.2%hold that the training base should pay for the salary and rewards of graduated college students who accepting training, and 73.4%thought the working unit of the resident physicians who accepting training should pay for the salary and rewards, there was no statistic difference among the different groups (P> 0.05).Conclusion:Human resource management system is a supporting system of the medical specialist training system, involving the personal arrangements, the management organization, the training cost, the compensation designing and management, rewards and disciplinary on specialist, and etc. Some active measures should be taken to establish a scientific, perfect and high efficient human resource management system.With the experience of other countries or regions and the results of the investigation, combining the present situation of residency training in Tianjin, it is necessary to establish the Accreditation Council for Graduate Medical Education to be responsible for the implementation of medical specialist training, the federate members of the council can include medical association, health administrative departments authorities and medical colleges and universities. It has more necessity to carry out the reasonable planning of human resource management, on the basis of the health service demands, the level and scale of medical education and regional health planning, including the position classification, quantity, financing modes, and so on. Meanwhile, the establishment of perfect assessment mechanism is very important too.
