| ObjectivesTo investigate and analyse the removable partial denture care and cleaning conditions among the middle-aged and the elderly wearers in Cities of Fuzhou,Quanzhou, maintenance of the knowledge of their dentures, denture cleaning behavior survey, to explore the impact of factors of denture cleanliness; to compare the effects of different denture cleaning methods and different cleaning agents, to discuss the cleaning effect of new denture cleanser - glycine-type amphoteric surfactant (Gly) surfactants, in order to make the targeted guidelines for dental care and continue to strengthen oral health related to the concept of general health, and do our best to improve the oral health of this population and to provide general health front-line information.MethodsThe study sample comprised 480 denture wearers in Cities of Fuzhou,Quanzhou through multi-stage, stratified, isasteric, randomize sampling.The denture status and the oral health status were examined for all subjects ,the oral questionnaire was also followed. From all subjects 60 patients were randomly selected. We experimented with their denture after obtaining their informed consent. by measuring The different cleaning methods and cleaning agents were evaluated by measuring their microorganism removal rates in denture plaque.Descriptive tatistics-Frequencies, Crosstabs, Kruskal-Wallis, Mann-Whitney, Crorrlate, Compare Means, One-Way ANOVA were performed to study the states and the influencing factors.Test level is 0.05.Results82.71% did not achieve the effective cleaning in Cities of Fuzhou,Quanzhou among the middle-aged and the elderly wearers. The detection rates of abnormal oral mucosa and denture stomatitis were up to 66.67% and 47.08%, both the detection rate were higher in men than women in rural areas than urban.Denture cleanliness of women is better than men, the urban is better than rural areas. Correlation analysis, obtained denture cleanliness of the relevant factors (Spearman correlation coefficient by the absolute value of descending order) were: denture wearing time and cleaning methods, and type of medical institutions, level of access to knowledge, getting off habits,smoking habits and level of education.In the study of the effect of different denture cleaning methods, the clearance rate of the combined treatment method of glycine salt or Polident with mechanical brushing was up to 100%. In a simple chemical treatment, The Candida and overall clearance rate of the glycine salt were better than Polident (P <0.05).Different kinds of microorganisms on the sensitivity of the same cleaning methods are different.ConclusionsThe situation of the denture cleanliness of the middle-aged and the elderly in Cities of Fuzhou and Quanzhou is worrying. Most of the patients wearing dentures were lack of the knowledge of denture cleaning and maintenance,and had poor denture cleaning and maintenance habits. Denture cleanliness subject to many factors.The different denture hygiene methods and different denture cleansers adopted in this study exhibited varying efficacies,in which the combination chemical and mechanical removal treatment with glycine salt or Polident achieved the best cleaning effects. In the simple chemical treatment, the cleaning performance of the glycine salt was better than Polident, it could remove Candidas effectively. Glycine salt amphoteric surfactant is a new type of denture cleaning agents and should promote its use in clinical. It is necessary to increase preventive health care and propagate oral health knowledge of removable dentures among the middle-aged and elderly to improve the denture cleanliness and promote their oral health.
