ObjectiveMetabolic syndrome (MS),which including multiple risk factors for cardiovascular, such as central obesity, elevated blood pressure,blood glucose levels, blood lipid abnormalities, is closely related to the pathogenesis and mortality of cardiovascular and kidney diseases. While, at present, the prevalence of MS in Meizhou,is still unknown. So this study is to ivestigate the prevalence of MS in Meizhou by collecting clinical datas of adults more than 20 years old,and analyze the influence of different diagnostic criteria to prevalence rates of MS in Meizhou.Method1176 Meizhou permanent residents were randomly selected from Affiliated hospital of Jia Ying medical college form January 2009 to January 2010, including 669 males,507 females. Medical examinations, age, gender, diabetes and hypertension, investigation of the history of gestational diabetes, as well as height, weight, waist circumference, blood pressure, triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), glucose and other indicators were checked. MS was diagnoses analyzed statistically according to NCEP-ATPⅢ(modification of the National Cholesterol Education Program Adult Treatment PanelⅢ,2005), IDF (International Diabetes Federation,2005)and CDS (Chinese Diabetes Society,2004) three standard diagnostic MS, and analyzed statistically. And using SPSS 13.0 software for statistical analysis.Results1. According to ATPⅢcriteria, the prevalence of MS was 14.3%(male 15.2%, female 13.0%).According to IDF,the prevalence is 9.7%(male 10.3% female 8.9%); while according to the standard of CDS, it is 9.2%(males 9.0% and females 9.5%). The statistical ruslts show that differences of prevalence between men and women are not significant (p> 0.05).2. But indicators of obesity and hypertension are closely related to MS. The prevalence related to obesity is 34.2% according to ATPⅢand IDF, and 32.9% according to CDS, The prevalence related to hypertension are 42.1%, according to the standard of ATPⅢand IDF, and 27.3% according to the standard of CDS.3. This survey also reveals that the incidence of people expressing one, two, three,four or five factors of obesity, high blood pressure, increased blood glucose, increased TG, decreased HDL-C is 32.7%,21.7%,10.7%,2.0%,1.0% sepreately according to the standards of ATP III and IDF; that is to say the incidence is 68.1%,35.4%,13.7%,3.0%,1.0% when at least one or 2,3,4,5 risk factor is expressed. In according to the CDS standard, the incidence of people expressing one, two, three,four or five factors above,incidence of MS is 31.1%,16.8%,8.7%, 1.8%,0.8%; That is 59.2%,28,1%,11.3%,2.6%,0.8% when at least one or 2,3,4,5 risk factors is expressed.4. The incidence of MS increased obviously with the age according to all of the three standards. The sub-paragraphχ2 test ruslt p<0.01, indicating that the prevalence were significant differences among different periods of age.The incidence of MS in people more than 70 yeas old is highest according to ATP III,while it is from 50 to 59 accroding to IDF and from 60 to 69 according to CDS.5. The incidence of MS is also related to gender.According to the three kinds of diagnostic criteria, before the age of 50, incidence of males is higher than females; and in group of people over 50 ages, incidence of females is higher than males.6. According to all the three standard,almost 6.4% of people are diagnosed with MS, while 85.7% diagnosed as non-MS.7. The results dignosed by the three diagnostic standards are consistently. The consistent rate between ATP III and IDF is 95.4%,Kappa value of 0.784 (males 0.780, females 0.788); It is 94.4% between CDS and ATP III,Kappa value 0.731 (0.666 for men, women 0.823); and 93.9% betweenCDS and IDF, Kappa value of 0.642 (males 0.563, females 0.751).8. Compared with the results in home and abroad, prevalence rate of MS in Meizhou is higher than Taiwan, Vietnam and South Korea, while lower than India, the Philippines, Hong Kong and Singapore, especially in Europe and America. the prevalence of MS in Meizhou is lower than that in northern areas, and generally balanced with other regions in South China.ConclusionThis study shows the high prevalence of MS of adults more than 20 years old in Meizhou, Guangdong Province of China. These results suggest that the region need to develop appropriate healthy strategies for prevention, detection and treatment of MS to reduce the social burden induced by cardiovascular diseases.
