Special ity:cl inic of integrated traditional and western medicine(pediatrics)Author:Li Li HuaTutor:Professor Chen Xiao FengBackgroundRecurrent respiratory tract infection(RRTI) is the most common disease in pediatrics;the occurring rate has gone up these years.Due to natural of disease:reiteration and prolong healing process,it badly affects children' s health.It is believed that various reason cause the disease in modern medicine.The triggering factors mainly relate to low immune system or disorder or poor nutrition.The Chinese medical professions believe that RRTI is caused by Qi-deficiency corporeity which catches evil factor;the key triggering factor is not due to too much evil but insufficient positive spirit. However,there is only few research on studying relationship among RRTI and weak corporeity and immune system;there is a need for further study on relationship between Qi-deficiency corporeity and immune system.PurposeTo explore the two types of T lymphocyte corporeity,characteristics of immunoglobulin and its clinical practices,through investigating the types of Qi-deficiency corporeity,other types of T lymphocytes corporeity and immunoglobulin.Furthermore,to seek for objectives based evidence in Chinese medicine,and also provide theory evidence for diagnosing,curing and preventing pediatric RRTI.MethodSet up the inclusion and exclusion criteria;based the diagnosis,select the pediatrics with RRTI who meet the criteria of the study;divide them into two groups according the corporeity according the Chinese medicine;test the level of peripheral blood T lymphocytes(CD3+,CD4+,CD8+,CD4+/CD8+), immunoglobulin(IgA,IgG,IgM),complement C3,C4 and microelement Zn; compared the result with the normal control group.Use SPSS 13.0 to analyze the data.Resultl.The child who diagnosis RRTI,majority is those with Qi-deficiency corporeity(58%).2.Those children with Qi-deficiency corporeity has the lower level of IgA,IgG,CD4+,CD8+,C3 and Zn comparing with normal control group,but CD8+ higher than the normal control group which has the significant difference (P<0.05 or P<0.01).the level of IgA,IgG,CD4+/CD8+,CD8+,C3,and Zn is not significant difference comparing those Qi-deficiency corporeity with the other types corporeity.3.Those with Qi-deficiency corporeity has lower CD4+ comparing with other types of corporeity.It has significant difference(P<0.05).4.Qi-deficiency corporeity has a relationship with the expression of IgA, CD4+,CD4+/CD8+Conclusion1.Based the analyzing corporeity of pediatric with RRTI,majority belongs to Qi-deficiency corporeity.2.T lymphocyte(CD4+,CD8+,CD4+/CD8+,immunoglobulin(IgA and IgG), complement C3 and microelement Zn in those RRTI pediatrics with Qi-deficiency corporeity type and other types of corporeity has certain disorder comparing with the normal control group.3.There is not enough evidence support to classified the types of corporeity according T lymphocytes(CD3+,CD8+),immunoglobulin(IgA and Ig G),complement C3,microelement Zn level.Expression of CD4+ is useful to distinguish between Qi-deficiency corporeity with other types;it serves as a reference indicator to recognize the RRTI Qi-deficiency corporeity. However,it needs further study to prove whether CD4+ can work as evidence guideline to detect the types of RRTI corporeity.4.RRTI Qi-deficiency corporeity symptom-complex integrate relates to the level of IgA,CD4+and CD4+/CD8+,it can serve as a guideline in clinical practice to diagnosis the Qi-deficiency corporeity RRTI based on the level of IgA,CD4+ and CD4+/CD8+;it is also helpful to check RRTI condition by comparing the level of IgA,CD4+ and CD4+/CD8+ before and after treatment. In conclusion,Qi-def iciency corporeity symptom-complex integrate can work as a diagnosis guideline to test the effectiveness of treatment for RRTI pediatrics,and it is easy to use.
