With the rapid progress of science and technology, the potential care security problems occurring in ICU also increased accordingly. In order to improve the nursing quality for the critically ill patients, ensure the patients safety and improve the patient's satisfaction, the admittance management of ICU care skills is implied in many hospitals. The implementation of the admittance management of ICU care skills is the warranty to improve the operating performance of the care skills, and which is also an important verification indicator of the effective admittance management for ICU care skills. Investigation of admittance management and performance analysis for ICU care skill is necessary. Than the problem in the admittance management for ICU care skills can be discovered in time. Investigation of admittance management and performance analysis can offer a reference to the admittance management for ICU care skills.OBJECTIVES:The syudy made a through understanding of the current status admittance management and performance for ICU care skills. Through the analysis to the operating performance under existing ICU admittance managemen system, further amending and improving admittance management system for ICU care skills. The problem in the admittance management can be discovered in time. Investigation of admittance management and performance analysis can offer a reference to the admittance management for ICU care skills. It can improve the nursing quality for the critically ill patients, ensure the patients safety and improve the patient's satisfaction.METHODS:Through literature searching, interview to the nurse and Delphi method, the questionnaire is formed. This study applied the multi-center stratified sampling method, conducted a questionnaire survey to the 38 ICU nurses in employment in 15 hospitals of Beijing and ones who has more than three years working experience in ICU with vice-senior title. The data about the admittance management and performance for ICU care skills was closed to be analyzed.RESULTS:1. In the 16 ICU based operation projects, various technical staff in technical operations there is no significant difference (P> 0.01 or> 0.05), of which 12 in operation performance there is no significant difference (P> 0.01 or> 0.05). It shows that in the operation of these projects there is a better effect of the implementation of operational admittance management so as to meet the needs of clinical care. 4 items in the operating performance have significant difference (P <0.01, or <0.05): sterile technique in the aspect of one-time success and quality of completion of care and technical operations, in the proficiency in the operation of skin and subcutaneous injection, in reducing operation Mistakes of intravenous infusion have differences. Their performance may be related to human resources, clinical practice and experience, knowledge and other relevant factors, so the key to the enhancement of operational management lying in solving the lack of manpower, strengthening and improving the clinical training and raising awareness of sterile.2. Survey results showed that in 31 specialist technologies, a total of 8 projects in the independent operation, various technical nursing staff were significantly different (P <0.01, or＜0.05), indicating that in this eight technical operations, some nurses did not meet admittance standards. And in this eight technical operations, in the different dimensions of performance, various technical nursing staff were significantly different (P <0.01, or <0.05), indicating it is necessary to operated admittance management. In 23, specialist technologies which various technical nursing staff have no different (P> 0.01 or> 0.05) in the independent operation, 6 of them in various technical nursing staff admittance have no difference (P> 0.01 or> 0.05), indicating in the operation of these projects there is a better effect of the implementation of operational admittance management. 17 of them in various technical nursing staff admittance have no difference (P <0.01, or <0.05) these are the items through admittance training and performance management but there is difference in the operation of projects, it may be security risks of the operation of care project.3.Survey results showed that in 31 specialist technologies, a total of 25 projects in the different dimensions of performance, various technical nursing staff were significantly different (P <0.01, or <0.05), indicating Of which 75 percent of the projects are innovative technologies that has only a technical training through short-term admittance to clinical application can not fully meet requirements. 4. In the five dimensions of the care skill performance, the different projects in operational proficiency and operational errors are the most. One-time success and complete effect follows, which is nursing is the main subject with clinical practice. Different levels' nurses' technical operation performance and clinical practice experience and ability are in direct proportion.CONCLUSIONS1. The implementation of admittance management to the basic operational projects has better results.2. Specialist technical operations admittance management system is to be refined; nurses need to implement stratified admittance management.3. The innovative technologies will be emphasis in the admittance management for ICU care skills.4. Through assessment to the different dimensions of performance, it becomes a necessary to improve the comprehensive training.
