Investigation And Study On Admittance For ICU Care Skills

Since China's entry into WTO,the admittance system gradually introduced into our country from abroad.The admittance management to health technology is to ensure good quality management.March 2,2009,the"clinical application of medical technology management"was issued by the Ministry of Health of People's Republic of China.On the notice,admittance to medical technology and medical management of classification and grading norms further strengthen the clinical application of medical technology management,regulatory systems in medical technology and improved admittance and management system.ICU(Intensive Care Unit,ICU) is a kind of department which serves for the critically ill patients and advanced monitoring rescue equipment and nursing staff. While it is also a high-risk medical establishment.With the rapid progress of science and technology,the potential care security problems occurring in ICU also increased accordingly.The implementation of the management of ICU care and admittance technology is the Warranty to improve the operating results and efficiency of the care,and which is also an important verification indicator of the effective management for technology admittance.Technology admittance management has become an important part of decision-making for the world health.The admittance for ICU care skills management in our country is still in its infancy.So, It is necessary to analyze the admittance for ICU nursing skills. OBJECTIVES:1.To understand the status quo about admittance management for ICU care skills,and the influencing factors about the level of care and technical operations.2.To explore the scope and classification methods of ICU care skills,and explore the standards of admittance to care and technology.3.To Analyze the actual technical level and the overall technical level which experts expectedMETHODS:The methods of this study included the Literature Search Method,Delphi Method,Questionnaire and Expert-structured interviews.We conducted one questionnaire survey to 30 ICU nurse specialists.Using random sampling method, we conducted another questionnaire survey to 849 ICU nurses of 79 ICU in 22 hospitals of Beijing.Through the investigation and understanding of the status of operational management of ICU care and admittance and through the analysis of results,the writer making a further understanding of the admittance management for ICU care skills,.and the standards of admittance to care and technology.RESULTS:1.Survey showed that the experts had more in-depth knowledge andunderstanding than ICU clinical nurses.All of the experts understood the concept of the admittance for ICU care skills,while only 70 percent of clinical nurses known this concept.Most of the ICU experts and clinical nurses considered that the basic and special care quality would improve with the implementation of technology admittance system.Most of the ICU experts and clinical nurses also thought that training was the effective way to improving the level of ICU care skills. 2.ICU experts and clinical nurses agreed that the most important factors to influencing the level of ICU care skills were operating frequency and training.3.This study identified 99 ICU care skill projects.ICU care skills can be divided into four categories according to the nature of nursing skills:basic care skills, intensive care skills,operations with medical,and interpretation of medical indicators.It also can be divided into three categories according to the difficulty:the first ICU care skills,the second ICU care skills,and the third ICU care skills.4.The expectation of ICU experts was much higher than the actual level of most of clinical nurses.CONCLUSIONS1.The admittance management of ICU care skills was started in Beijing,but it was still in its infancy.The most important ways about the admittance management were training and induction by professional and technical positions.2.Operating frequency was the most important factors to improve ICU care skills.Training evaluation can effectively increase the level of ICU care skills3.The first step to carry out the admittance for ICU care skills was to start from basic nursing skills.4.Health management and health care units can develop training programs according to the features of three characteristics of ICU care skills.5.According to the status for mastering ICU care skills,the majority of nursing staff at all technical levels did not meet the level of ICU experts'expectation.
