| Objective To understand the current situation of Health human resourcein Hunan Province by analyzing its quantity, quality, structure, allocationand distribution, to lay a foundation for the reasonable allocation ofhealth resources and regional sanitation layout and to offer the scientificbasis for decision-making for health administration, medical institutionmanagement.Methods We made the best of Hunan economical annual review, Hunanhealth human resource database and the information of hunan healthmanpower status investigation of 2004, and then established the data-basethrough the Excel. All data derived from questionnaire were processed bySPSS software package (version 13.0) for descriptive statistic andchi-square test.Results1. The total amount of medical health personnel and distribution: By2004 the number of Medical Health Personnel (MHP), Medical HealthTechnological Personnel (MHTP) and administrative personnel were194172, 183390 and 11878 respectively. The allocation of healthmanpower was significantly different in different region for all the 14cites. In terms of the total amount of medical health personnel Changsha,Hengyang, Changde, Shaoyang were ranked the first four position,Zhangjiajie was ranked in the last place, however, on the aspect of theamount of medical health personnel per thousand people, Changsha ledthe whole 14 cities for 3 years in succession (6.03-6.56), Shaoyang hadthe lest amount, the average was 3.72-3.84 for the province. MHP weremainly distributed in comprehensive hospitals, account for 40.35ï¼…, thefollowing was rural public health center, accounting for 31.90ï¼…. Amongall the MHP female and Han people took the majority. Age distribution was centralized between from 20 to 54. Regarding the educationbackground of MHP, the majority was college graduates and vocationalschool graduates, the proportions were 27.06ï¼…and 47.56ï¼…respectively.MHP with high educational background was insufficient.2. The amount of Medical Health Technological Personnel (MHTP)and distribution: Among 14 cities Changsha had the largest amount ofMHTP per one thousand person, that was 4.76-5.19, the second rank wasXiangtan and Zhuzhou, Shaoyang had the smallest amount, that was2.21-2.28. MHTP were mainly distributed in comprehensive hospitals,amount to 39.81ï¼…, the following was rural public health center (32.87ï¼…)and traditional Chinese hospitals (10.39ï¼…). MHTP mainly worked insunch departments as medica dept (16.07ï¼…), surgical dept (8.38ï¼…),general praction dept (6.73ï¼…), department of gynaecology and obstetrics(6.23ï¼…) and traditional Chinese medicine dept. The amount of femaleMHTP overtopped the male. Age distribution was centralized in the 30group, amount to 21.11ï¼…. The specialization for MHTP mainly wasnursing and clinic medicine, tant was 25.96ï¼…and 20.09ï¼…respectively.3. The amount of Medical Health Administration Personnel (MHAP)and distribution: The total amount was 10782 in Hunan, Chansha,Shaoyang, Changde occupied a large proportion, they were 18.18ï¼…,14.50ï¼…and 10.14ï¼…respectively. MHAP were mainly distributed incomprehensive hospitals, amount to 49.42ï¼…, the following was ruralpublic health center (15.50ï¼…) and traditional Chinese hospitals (11.55ï¼…),various special hospitals (6.48ï¼…), hygienic and antiepidemic station(4.73ï¼…) and Maternal and Child Health hospitals. There were 70.63ï¼…ofMHAP from administrative dept, 17.17ï¼…from other professional depts.and 4.96ï¼…from CDC. Age distribution was centralized in the 35 group,45group and 30 group, amount to 18.21ï¼…, 17.42ï¼…and 16.69ï¼…respectively. The specialization for MHTP mainly was nursing and clinicmedicine, tant was 25.96ï¼…and 20.09ï¼…respectively. The number of thefemale surpassed that of the male. Han people took the majority. Theproportion of professional title of MHAP was mainly descendingly as follows, politicers 0.58, all of the associate chief physician, attendingphysician and resident physician account to 12.44ï¼…, charge nurse 4.82ï¼…,there were 26.60ï¼…of MHAPs whose professional title was unknown.Regarding the education background of MHAP, the majority was collegegraduates and vocational school graduates, the proportions were 32.59ï¼…and 27.55ï¼…respectively, the proportion of senior middle school was aslarge as 19.07ï¼…, undergraduate was only in a proportion of 11.72ï¼…,postgraduate and doctor were scarce.4. The allocation of health resource: The bed for per one thousandperson increased year by year from 2.13 in 2002 to 2.18 in 2003 then to2.21 in 2004. Changsha had the highest ratio among 14 cities (3.68-3.99),the following were Xiangtan and Zhuzhou, during the 3 years Shaoyangincreased in the lest rate. The average ratio of certificated doctor to bedwas 0.40-0.41, the average ratio for RN to bed was 0.42-0.43.Conclusion The distribution of health manpower was uneven in urbanand rural areas, and the structure of health manpower was unreasonable,and the general diathesis of health manpower was in a low level. So thatthe health manpower can not keep in coordination with the developmentof health industry in Hunan. The total amount of medical health humanresource was insufficient, especially, the practitioner doctor andRegistered Nurse were badly in short, the ratio of the practitioner doctorand Registered Nurse was unreasonable. |
