Background Gastric mucosa damage is a common clinical disease, including the stomach inflammation and gastric ulcer etc, and is usually considered to be out of balance of the two factors (attacking factor and defending factor ) in the gastric mucosa. In the aspects of medicine curing gastric mucosa damage, Chinese herbal has played an important role in treating such kind of disease for many years, the Jian Wei Yi Qi Tang (GWYQT) is observed to have the good protect effect with the gastric ulcer and the stomach inflammation on clinic, but its mechanism is still not quite clear.Objective The model of gastric mucosa damage on rat was established by giving absolute alcohol into stomach, the protect effects of GWYQT to alcohol-induced gastric mucosa damage were studied histologically. The change of apoptosis in gastric gland cell between alcohol-induced gastric mucosa damage group and GWYQT protect group was observed. And the mechanisms of GWYQT protect effect to alcohol-induced gastric mucosa damage were investigated. This study can provide scientific theories and experiment dates for the clinical use of GWYQT to treat the diseases of gastric mucosa damage.Method 1.Establish the animal model: Forty Wistar male rats were randomly assigned into 4 groups as follows, GWYQT;Western medicine control;experiment control;normal control. GWYQT group was given GWYQT(10ml/ kg), western medicine group was given De-Nol liquid (10ml/ kg), two controls were given NS in the same volume. All drugs were administrated intragastrically once a day for 4 days. All rats were fasted for 24h but were given free access to water before sacrifice, 30 minutes after the last administration, ex-three groups were given absolute alcohol into stomach(1mg by one) respectively and were sacrificed together with normal control group by cervical dislocation under light ether anesthesia, and a middle laparotomy was performed after 1h. The total stomach was removed quickly. 2. Histologic observation: the stomach mucosa damage index were measured. The gland stomach tissues was taken and Paraffin sections were stained with H-E and PAS for histologic observation by LM. 3. Stained with the SABC method of immunocytochemistry, to show the expression of apoptosis related proteins Bcl-2/Bax in the mucosa of the rat stomach. Image analysis was used to detect the expression of these proteins and toclarify the correlation between the expression of BcL-2/Bax proteins in gastric gland and gastric mucosa damage, and to study the influence of the GWYQT to apoptosis of gastric mucosa.Results Except the normal control group, absolute alcohol lead to multiple gastric ulcerations in all animals. However, the frequency, number, and size of gastric ulcers were significantly reduced both in GWYQT group and western medicine group rats, especially in the GWYQT group animals. The damage index(26.87±6.84)in the GWYQT group is significantly different (P<0.01) from the experiment control group(98.33ifc 15.36). With LM, experimental control animals displayed a loss of the upper mucosa and many inflammatory cells were observed in H-E specimens. In striking contrast, gastric specimens from the GWYQT pretreated animals display minimal or almost no changes and are significant different (P<0.01) from the experimental control group. Moreover, the thicker and more complete gastric mucus layer were observed in the GWYQT pretreated rats than in experimental control group in the Alcian-PAS specimens (P<0.01). In detecting and analyzing the expression of apoptosis related proteins Bcl-2/Bax in the mucosa of the rat stomach by immunocytochemistry and Image analysis, our finding reveal that the expression of Bax protein is decreased significantly in the gastric mucosa, and Bcl-2 expression is increased in the GWYQT pretreated rats, which is significantly different (P<0.01) from the experiment control group.Conclusion l.The experiments show that GWYQT can decrease the damage index, reduce the number of inflammation cells, increase thicknesses of gastric mucus layer, which indicated that GWYQT have good anti-inflammation, anti-gastric mucosa damage, and strengthen stomach musoca protection. 2.The experiments also show that GWYQT can descend the expression of Bax, and ascend expression Bcl-2, increase Bcl-2/ Bax, repress the apoptosis, alleviate gastric mucosa damage. 3. GWYQT have a good protective effects to gastric musoca damage, and is better than the Western medicine, and it probably can be used in clinic after further studies.
