Objective: To assess the efficacy and safety of noninvasive positive pressure ventilation ( NIPPV ) for acute exacerbation of chronic obstructive pulmonary disease (AECOPD) patients on general wards. Methods: A trial was conducted in the second affiliated hospital,ZJU between 2005.11 and 2006.2. 40 AECOPD patients were recruited ongeneral wards and assigned to standard medical treatment (group A, n= 25) or early administration of NIPPV plus standard medical treatment(group B, n = 15) .Results: The characteristics of the two groups on admission were similar. Arterial partial pressure of oxygen (PaO2) and arterial partial pressure of carbon dioxide (PaCO2) improved significantly in group B. The mortality was no significant difference between the two groups. Conclusions: The early use of NIPPV on general wards improvesarterial blood gas and respiratory pattern0 NIPPV is indicative for alleviating respiratory muscle fatigue and preventing respiratory failure from exacerbation,,...
