The Palliative Operation Of 268 Cases With The Advanced Colorectal Carcinoma

objective: To study the meaning of the palliative operation in the patients with the advanced colorectal carcinoma which can not be radical cure and the option of the operation method. Methods: 268 patients with the advanced colorectal carcinoma underwent the palliative operation from 1989 to 1999 were analyzed retrospectively. Results: 1 Exploratory laparotomy: The group have 13 cases. It can not resect the tumor, ameliorate the symptom ,lighten the suffering ,and perfect the quality of life. Moreover it can add the new injures and complications ( 13.04% ) to the patients. Meantime it has a bad prognosis, the patients died in 2 months after this operation. 2 Stomasy operation: Our group has 63 cases. This method include the side-side anastomosis and sigornoidosyomy. In the 14 cases underwent the side-side anastomosis and the 49 cases underwent the sigornoidosyomy, the tumor has not been resected, the average survial span is only 3 months. But this method can relieve the block, lighten the suffering, ameliorate the symptom, and improve the quality of life. But the sigornoidosyomy made the new pains to the patients, and the stomasy operation can not prolong the survial span. 3 The palliative resection : it can ameliorate the symptom, relieve the suffering, improve the quality of life, and relieve the charge of the tumor. Meantime it can give the chance to the further treatment, prolong the survial span. The Hatermann's operation has important signation in this method. 4 Expanding palliative resection: it can relieve the symptom. But due to the large injury of operation, the complications is more ( 42.86% ) and serious moreover. It's survial span and the rate of lightening the suffering has no differences to palliative resection. Conclusion: 1 The palliative resection is the best select to the patients with the advanced colorectal carcinoma which can not be radical cure. But expanding palliative resection has no advantage. 2 Stomasy operation is a good option to the palliative treatment. 3 In the palliative resection ,we should attach importance to the Hartmann's operation.
