Objective:Due to the anatomical location and pathological characteristics,the right colon liver curvature carcinoma has a high recurrence rate,and more common is the lower pyloric lymph node metastasis.Whether to perform subpyloric lymph node dissection during radical resection of right hemicolon cancer and its indication are an important clinical problem for many general surgeons.Perioperative efficacy method of comparing D3 radical resection and complete mesiectomy for colorectal tumors compared by Meta-analysis: Pub Med was mainly retrieved by computer,EMBASE Cochrane Library,Sciencedirect,The National Internet Network of China(CNKI),Wanfang,VIP(CQ-VIP),and other Chinese and English databases,To find out the relevant clinical studies comparing the efficacy of radical D3 surgery to complete mesonectomy for colorectal tumors(as of December 2019),Two researchers independently screened the literature,evaluated the quality of the literature,and extracted the relevant index data,Finally,the pooled statistical analysis was performed using the Rev Man 5.3 software.Results:A total of 13 clinical studies(4 RCT,9 NRCT)were included,and 1732 patients were.According to comprehensive analysis,the final results were as follows: the duration of the two procedures(P=0.12),the number of intraoperative lymph node dissection(P=0.37),while the amount of bleeding in radical D3 surgery was less(P<0.000001).In terms of postoperative effect,D3 had early initial exhaust time(P=0.003),reduced postoperative pain(P<0.00001),higher postoperative appearance satisfaction(P <0.00001),shorter postoperative hospitalization(P<0.0001),and lower overall postoperative complications(P=0.0009),with decreased ileus and incision infection(P=0.02;P <0.0001),and no significant difference in repeated anastomosis and intraabdominal infection(P=0.84;P=0.49).Three-year and 5-year disease-free survival rates were not significantly different(P=0.56;P=0.74).
