Objective To compare the efficacy and survival of Middle and late stage pancreatic head cancer patients with endoscopic stenting or palliative bypass surgery.Methods By retrospectively analyzing the date of Middle and late stage pancreatic head cancer patients,who were treated with palliative treatment in General hospital of Ningxia Medical University from January 2011 to June 2016,including 107 cases of endoscopic stent implantation and 29 cases of palliative bypass surgery,and following up the survival rate.Then compare the levels of TBil,DBil,ALT,AST before and after operation,postoperative complications,postoperative fasting time,hospitalization time,hospital costs,abdominal pain and Karnofsky quality of life improvement and postoperative survival in the two groups of patients,who were treated with endoscopic stent implantation or palliative bypass surgery.Results The serum levels of TBil,DBil,ALT,AST were significantly decreased in the two groups after three days of the operation(P<0.05),but the differences between the two groups before and after surgery were not statistically significant(P>0.05),so was the incidence of postoperative complications and hospital costs(P>0.05).The postoperative fasting time and hospitalization time between the two groups were of statistically significant differences(P < 0.05).Three days after surgery,Karnofsky quality of life score and abdominal pain relief rate were significantly higher in the stent group than in the surgery group,with the difference of statistically significance(P <0.05).The cumulative survival rates for the 3 months,6 months,and 12 months in the stent group and the surgical group were 77.1%,38.6%,15.7%,and 84.2%,52.6%,21.1%,respectively,with the difference of no statistical significance(P>0.05).Conclusion Endoscopic stent implantation and palliative surgery have the same effect of early drainage and reducing jaundice in Middle and late stage pancreatic cancer patients,but endoscopic stenting has the advantages of shorter postoperative fasting time,faster recovery,higher remission rate of abdominal pain without increasing hospitalization costs,and it also improve patient’s postoperative quality of life.
