BACKGROUD and OBJECTIVE Chronic obstructive pulmonary disease (COPD) is a common disability, largely encountered in the elderly population, in whom it causes significant morbidity and mortality. Though the noninvasive ventilation (NIV) has been shown to be effective in patients with respiratory failure due to acute exacerbations of chronic obstructive pulmonary disease (COPD), the complications cause by NIV restrict its application. There is no perfect ventilation project up to day. The aim of this study is to observe the effects of Precipitation therapy of Traditional Chinese Medicine (TCM) combined with NIV on gas exchange and patients' transformation in the duration of ventilation, the reintubation rate and the rate of ventilation-associated pneumonia, and to evaluate their clinical value.METHODS Fifty four patients with respiratory failure due to acute exacerbations of COPD were randomly divided into study group (28 cases, treat by Precipitation therapy of TCM combine with NIV) and control group(26 cases, treat by NIV). All patients received routine treatment. Arterial blood pH, PaCO2 and PaCO2 were measured and heart rate were recorded in both groups. The incidence of ventilator associated pneumonia(VAP), the incidence of death, the reintubation rate, the duration of ventilation were analyzed in two groups.RESULTS There were no difference in clinical characteristics and gas exchange at entry.
