| Objective:Through AIDS 1 combined with HAART adverse immune reconstruction in patients with AIDS clinical research and exploration for low immune model mice experiment,to observe the party reflects the kidney’s poison treatment on immune reconstruction bad feasibility,to further explore the mechanism of traditional Chinese medicine on immune reconstruction,in order to solve HAART immune reconstruction after bad problem to provide ideas and methods.Methods:Based on clinical experience,theory and literature research methods,to explore the pathogenesis hypothesis of TCM disease name and immunoreconstitution of AIDS,put forward the corresponding treatment methods and elaborate their theoretical characteristics,and improve the formulation of TCM formula 1 for AIDS.Using the prospective before and after self-controlled clinical study method,10cases of patients with poor HAART immunoreconstitution were conducted with the collaborative clinical intervention of Chinese and western medicine,and clinical symptoms and signs,laboratory indicators,immunological indicators and safety indicators of the subjects were observed to evaluate the clinical efficacy of AIDS formula 1 on patients with poor HAART immunoreconstitution.The immunocompromised mice model induced by cyclophosphamide and cyclosporin A was used to observe the effects of AIDS formula 1 on delayed hypersensitivity,organ index and peripheral blood CD4~+and CD8~+T lymphocytes in the model mice,and to evaluate the effects of this formula on immunocompromised mice.Results:Relevant theoretical exploration of traditional Chinese medicine(1)AIDS disease of traditional Chinese medicine"ai li"and causes"evil"can be used as an academic discussion;(2)"Aixie damage yuan,toxin and blood stasis endogenous"is the pathogenesis hypothesis of poor immune reconstruction of AIDS,among which aixie invasion is the cause of disease,qi deficiency is the basis of disease,toxin and blood stasis are both pathological products and pathogenic factors;(3)Tonifying the kidney and removing blood stasis is the basic principle of AIDS immune reconstitution.Tonifying the kidney is the foundation of treatment,tonifying the toxin is the means of treatment,and removing blood stasis is the purpose of treatment.Clinical research(1)The percentages of HLA-DR~-CD38~+CD4~+/CD8~+T cells and HLA-DR~+CD38~+CD4~+/CD8~+T cells decreased significantly at week 24 and12(P<0.05),The percentage of HLA-DR~-CD38~+CD8~+T cells decreased significantly at week 36 compared with week 12(P<0.05);(2)Effective rate of immune reconstruction:at the 24th week,1 case was shed,3cases were effective,6 cases were stable,and 0 cases were ineffective.At the 36th week of treatment,1 patient was exfoliated,4 effective,5 stable and 0 invalid;(3)CD4~+T cell count,CD8~+T cell count and CD4/CD8 ratio were increased at the24th and 36th weeks after administration compared with that before administration(0weeks)(P>0.05);(4)The percentages of CD4~+CD45RA~+/CD45RO~+and CD8~+CD45RA~+/CD45RO~+T lymphocytes fluctuated up and down at week 24,36 and 12(P>0.05);(5)The percentage of HLA-DR~+CD38~-CD4~+/CD8~+T cells showed no significant change at week 24 and week 12 after treatment(P>0.05),and the percentage of HLA-DR~+CD38~+CD4~+/CD8~+T cells showed no significant change at week 36 and week 12 after treatment(P>0.05);(6)After 12 weeks,24 weeks,36 weeks compared with before(0 weeks),the total score of symptoms and signs decreased(P>0.05).Karlovsky scores rose slowly(P>0.05).Animal testingEffects of formula 1 of AIDS on delayed hypersensitivity of normal and immunocompromised mice induced by cyclophosphamide:(1)Compared with the blank control group,the ear weight difference,thymus index and spleen index in the model control group were significantly reduced(P<0.05);(2)Compared with the blank control group,there were no significant differences in ear weight difference,thymus index and spleen index in the no.1 group(P>0.05);(3)Compared with the model control group,there was no statistical difference in ear weight difference,thymus index and spleen index between the no.1+model group and the positive control group(P>0.05).Effects of formula 1 of AIDS on immunocompromised mice induced by cyclosporine A:(1)Compared with the blank control group,ear swelling degree,CD4~+T cell amount and CD4/CD8 ratio in the model control group were significantly reduced(P<0.05);(2)Compared with the model control group,the ear swelling degree of mice in the no.1 large and medium dose group and the positive control group was significantly increased(P<0.01,0.05).There was no significant difference in ear swelling degree in the no.1 low-dose group(P>0.05);(3)Compared with the model control group,the amount of CD4~+T cells and CD4/CD8 ratio in each dose group of HIV no.1 were significantly increased(P<0.01),while the amount of CD8~+T cells was not significantly changed(P>0.05).Conclusion:The pathogenesis hypothesis of"aixie damage yuan,toxin and blood stasis endogenous"reflects the essence of AIDS immunoreconstitution.In the clinical exploration and animal experiment of this group,the no.1 side of AIDS showed the following phenomena:(1)It has the effect of down-regulating the subgroup of immune activation of T cells in patients with poor immune reconstitution,and can suppress the abnormal immune activation status of patients,which may be one of the mechanisms of this prescription to promote the immune function reconstruction of patients;(2)Can improve the clinical symptoms of AIDS patients,improve the quality of life,in cold,diarrhea,mental state,work ability and other aspects of the role is particularly obvious,good safety;(3)The level of delayed hypersensitivity induced by cyclosporin A in immunosuppressed mice was significantly increased,and there was A certain quantitative relationship between the therapeutic effect and dose.At the same time,it was able to improve the low expression of CD4~+T cells in immunosuppressed mice induced by cyclosporin A;(4)It did not show the effect of increasing CD4~+and CD8~+T lymphocyte count and restoring CD4/CD8 ratio in patients with poor immune reconstruction;(5)It did not show the effect of increasing the percentage of CD4~+CD45RA~+T cells,CD8~+CD45RA~+T cells,CD4~+CD45RO~+T cells,CD8~+CD45RO~+T cells in patients with poor immunoreactivity.It may be that the effect mechanism of AIDS no.1 on the change of pure and memory CD4~+T lymphocyte subpopulation in patients with poor immunoreactivity is not consistent with the situation of AIDS patients after initial HAART;(6)It had no effect on the immune function of normal mice and mice with immunodeficiency induced by cyclophosphamide. |
