BackgroundStress urinary incontinence(SUI)is a common disease,seen mostly in women.It is the involuntary leaking of urine associated with coughing,sneezing and exercising etc.that increase intra-abdominal pressure and thus increase pressure on the bladder.Worldwide,the incidence of urinary incontinence in women is 25.7%(3.8%-45.1%),and the incidence of SUI is as high as 50%.SUI patients in China occur mostly in women,and the incidence of the incontinence is 32.15%.It can be a common and distressing problem for women with SUI,which may have a profound impact on social activities,personal hygiene,and quality of life and in physical and mental health.Stress urinary incontinence belong to the "involuntary urination" category in TCM,and the usual cause of this disease is related to the deficiency of qi-blood,loss of control in the bladder,instability of kidney-qi,or damp invasion of the lower energizer leading to the gathering in the bladder postpartum damage etc.which leads to the dysfunction of the bladder,causes the leakage of urine.According to the recent clinical researches,acupuncture is proven to be quick and effective,and with the combination of electrical stimulation in the acupoints,it is able to maintain and increase the acupuncture sensation during clinical treatment.Our research is based on different acupuncture observation periods,to investigate the therapeutic effect of electro acupuncture in SUI patients.ObjectiveTo observe and evaluate the therapeutic effect of electro acupuncture in SUI patients in different acupuncture treatment periods.MethodOur research has selected female stress urinary incontinence patients since March 1st,2013 until September 3rd,2014 from Beijing Dongzhimen Hospital outpatient services as our object of study.Clinical research proceeded with 42 cases of patients who satisfies the inclusion criteria,ran through the clinical research central stochastic system and were randomly divided into two groups:Treatment group 20 cases(electro-acupuncture group)and control group 22cases(sham acupuncture group).Treatment group was treated with the addition of electrical stimulation in both Zhongliao(B33)and Huiyang(B35)acupoint;control group was treated with sham acupuncture 1 inch beside both Zhongliao(B33)and Huiyang(B35)acupoint,30 minutes per session,once every other day,3 times per week,total of 6 weeks of treatment.Our research have observed the following indexes:the severity of 72h urinary incontinence,the frequency of 72h urinary incontinence,the usage of 72h incontinence pads,1h pad test,the International Consultation on Incontinence Questionnaire-Short Form(ICIQ-SF),UDI urinary and reproductive exertion scale,visual analogue scale(VAS),frequency of enuresis nocturna and subjective evaluation of efficacy in patient.At last,observe the results between and within groups,to observe and evaluate the therapeutic effect of electro acupuncture in SUI patients in different acupuncture treatment periods.Result1.Different observation periods comparing the severity of 72h urinary incontinencein both groupsTreatment group:after 2 weeks,4 weeks and 6 weeks of treatment,the severity of incontinence D-value did not show any significant difference(P<0.05).Control group:after 2 weeks,4 weeks and 6 weeks of treatment,the severity of incontinence D-value were shown with no significant difference(P<0.05).Result indicates that the severity of incontinence in treatment group decreases as the amount of treatment increases;no significant difference was shown before and after treatment in control group.2.Different observation periods comparing the frequency of 72h incontinence in both groupsComparing between groups:after 2 weeks,4 weeks and 6 weeks of treatment,the frequency of incontinence in the treatment group showed significant increase in the difference(P<0.05);but the control group did not show any significant difference(P>0.05)Comparing within groups:1)treatment group:after 2 weeks and 4 weeks of treatment,the frequency of incontinence showed significant difference P<0.05),but in the difference between 4th and the 6th week,D-value did not show any significant difference(P>0.05).2)control group:after 2 weeks,4 weeks and 6 weeks of treatment,D-value did not show any significant difference(P>0.05).Result indicates that after 6 weeks of treatment in both groups,the frequency of incontinence in treatment group showed an evident decrease than the control group.Comparing within groups indicated that the frequency of incontinence in the treatment group showed an evident decrease and as the increase in frequency of treatment,the frequency of incontinence showed a decrease.The therapeutic effect in the 4th week was more evident than the 2nd week,but there was no significant difference between the 4th and the 6th week.There was no significant difference before and after the treatment in the control group.3.Different observation periods comparing the usage of 72h incontinence pads in both groupsComparing between groups:after 2 weeks,4 weeks and 6 weeks of treatment,the usage of 72h incontinence pads in treatment group showed significant increase in the difference(P<0.05);but the control group did not show any significant difference(P>0.05)Comparing within groups:1)treatment group:after 2 weeks,4 weeks and 6 weeks of treatment,the usage of 72h incontinence pads showed significant difference(P<0.052)control group:after 2 weeks,4 weeks and 6 weeks of treatment,D-value did not show any significant difference(P>0.05).Result indicates that after 6 weeks of treatment in both groups,the usage of 72h incontinence pads in treatment group showed an evident decrease than the control group.Comparing within groups indicated that the usage of 72h incontinence pads in the treatment group showed an evident decrease and as the increase in frequency of treatment,the usage of pads showed a decrease.There was no significant difference before and after the treatment in the control group.4.Different observation periods comparing amount of urine in 1h pad test in bothgroupsComparing between groups:after 2 weeks,4 weeks and 6 weeks of treatment,the amount of urine in 1h pad test in treatment group showed significant increase in the difference(P<0.05);but the control group did not show any significant difference(P>0.05)Comparing within groups:1)treatment group:after 2 weeks,4 weeks and 6 weeks of treatment,the amount of urine in 1h pad test showed significant difference(P<0.052)control group:after 2 weeks,4 weeks and 6 weeks of treatment,D-value did not show any significant difference(P>0.05).Result indicates that after 6 weeks of treatment in both groups,the amount of urine in 1h pad test in treatment group showed an evident decrease than the control group.Comparing within groups indicated that the amount of urine in 1h pad test in the treatment group showed an evident decrease and as the increase in frequency of treatment,the amount of urine in 1h pad test showed a decrease.There was no significant difference before and after the treatment in the control group.5.Different observation periods comparing ICIQ-SF scale scoring in both groupsComparing between groups:after 2 weeks,4 weeks and 6 weeks of treatment,ICIQ-SF scale scoring in treatment group showed significant increase in the difference(P<0.05);but the control group did not show any significant difference(P>0.05)Comparing within groups:1)treatment group:after 2 weeks,4 weeks and 6 weeks of treatment,ICIQ-SF scale scoring showed significant difference(P<0.052)control group:after 2 weeks,4 weeks and 6 weeks of treatment,D-value did not show any significant difference(P>0.05).Result indicates that after 6 weeks of treatment in both groups ICIQ-SF scale scoring in treatment group showed an evident decrease than the control group.Comparing within groups 2 weeks,4 weeks and 6 weeks of treatment,UDI scale scoring in treatment group showed significant increase in the difference(P<0.05);but the control group did not show any significant difference(P>0.05)Comparing within groups:1)treatment group:after 2 weeks,4 weeks and 6 weeks of treatment,UDI scale scoring showed significant difference(P<0.052)control group:after 2 weeks,4 weeks and 6 weeks of treatment,D-value did not show any significant difference(P>0.05).Result indicates that after 6 weeks of treatment in both groups UDI scale scoring in treatment group showed an evident decrease than the control group.Comparing within groups indicated that UDI scale scoring in the treatment group showed an evident decrease and as the increase in frequency of treatment UDI scale scoring showed a decrease.There was no significant difference before and after the treatment in the control group.7.Different observation periods comparing visual analogue scale(VAS)scoring in both groupsComparing between groups:after 2 weeks,4 weeks and 6 weeks of treatment,VAS scoring in treatment group showed significant increase in the difference(P<0.05);but the control group did not show any significant difference(P>0.05)Comparing within groups:1)treatment group:after 2 weeks,4 weeks and 6 weeks of treatment,VAS scoring showed significant difference(P<0.052)control group:after 2 showed a decrease.There was no significant difference before and after the treatment in the control group.8.Different observation periods comparing the frequency of enuresis nocturna in both groupsComparing between groups:after 2 weeks,4 weeks and 6 weeks of treatment,the frequency of enuresis nocturna in treatment group showed significant increase in the difference(P<0.05);but the control group did not show any significant difference(P>0.05)Comparing within groups:1)treatment group:after 2 weeks,4 weeks and 6 weeks of treatment,the frequency of enuresis nocturna showed significant difference(P<0.052)control group:after 2 weeks,4 weeks and 6 weeks of treatment,D-value did not show any significant difference(P>0.05).Result indicates that after 6 weeks of treatment in treatment in the control group.9.Different observation periods comparing the subjective evaluation of efficacy in both groupsComparing between groups:after 2 weeks,4 weeks and 6 weeks of treatment,the subjective evaluation of efficacy in treatment group showed significant increase in the difference(P<0.05);but the control group did not show any significant difference(P>0.05)Comparing within groups:1)treatment group:after 2 weeks,4 weeks and 6 weeks of treatment,the subjective evaluation of efficacy in patients showed significant difference(P<0.052)control group:after 2 weeks,4 weeks and 6 weeks of treatment,D-value did not show any significant difference(P>0.05).Result indicates that after 6 weeks of treatment in both groups the subjective evaluation of efficacy in treatment group showed an evident decrease than the control group.Comparing within groups indicated that the subjective evaluation of efficacy in the treatment group showed an evident decrease and as the increase in frequency of treatment the subjective evaluation of efficacy in patients showed a decrease.There was no significant difference before and after the treatment in the control group.Conclusion1.Stimulating Zhong Liao and Hui Yang acupoint by electroacupuncture can effectively regulate the detrusor urinae of bladder,urethral sphincter function and strengthen the self-control urination function.2.As the treatment frequency increases,treatment duration extends,the therapeutic effect increased more signiflcantly.In the 6th week of treatment,all the index were shown with significant improvement especially in the amount of urine leakage.3.As the symptoms of SUI improve,the ICIQ-SF score,SUI patient self evaluating score and the visual analogue scale(VAS)have improved simultaneously.4.The compliance of the patients was good with no adverse reaction and complication within 6 weeks of electro-acupuncture treatment in SUI patients.
