Recurrent oral ulcer is a frequently encountered disease. Splenogastric Hygropyrexia Syndrome is one kind of common syndrome for ROU. Studies on it is helpful to explore in the syndrome essence. But the animal model duplication is very important to Splenogastric Hygropyrexia Syndrome' pathogenesis, the new drug research and development and enhancing clinical curative effect. In this study, we try to find out the biology foundation and occurrence mechanism of Splenogastric Hygropyrexia Syndrome in the ROU by the way of VEGF, TNF-αexpression. At the same time, we observed the influence of KouKuiFang on index of correlation to Splenogastric Hygropyrexia Syndrome' rat model and clinical patients.1. Literature ResearchThrough the clinical and empirical research for ROU, we think that the pathogenesy is not extremely clear-cut. It is correlated to many factors such as inheritance, immunization, insufficient elements, infection, endocrine secretion, spirit and psychologic. The therapy of traditional Chinese medicine is multiformity, but the curative effect is not full unity. The method to build the ROU animal model is multiformity, but also has not appeared the animal model which is publicly accepted and duplicated.The modern research of Splenogastric Hygropyrexia Syndrome refer to many directions such as epidemiology, inflammation, immunization. The Splenogastric Hygropyrexia Syndrome animal model study are not many, but also has not appeared the standard model which is publicly accepted and duplicated. The ROU from the spleen and stomach treatment is related to cavity structure, physiological function, disease pathology of the oral.VEGF is a high degree of specificity growth factor to promote karyogenetic division of vascular endothelial cell and blood vessel osmosis. VEGF is important for ulcer reparation. VEGF which is a protection factor to promote the ulcer concrescence shows high express during repairing in trauma and ulcer. TNF which the mononuclear macrophage product is a kind of cell factor to possess generally biologic activity. TNF appears earlier period of the inflammatory reaction and induces many cell factors or some mediators of inflammation production. TNF has important action during the cell function regulation, immunization and inflammatory reaction. Though the research of VEGF and TNF-αare many, the study by the way of the ROU and Splenogastric Hygropyrexia Syndrome conjunction still do not be found. Therefore, it reveal the ROU essence of traditional Chinese medicine syndrome by researching the patho-physiological effect of VEGF and TNF-αin the pattern of syndrome. It will supply new methods for clinical prevention and cure.2. Clinic ResearchObjects:Choose 38 cases of patients with recurrent oral ulcer(ROU), 38 cases belong to the group of Splenogastric Hygropyrexia Syndrome. Another 6 cases belong to the group of Normal health examination people.Method:All observed object was drawn to venous blood in the morning. VEGF, TNF-αwas detected by the way of ELISA. Then all patients used oral medication of KouKuiFang for a week. The treatment group was drawn to venous blood again after a week.Results:The VEGF expression of Splenogastric Hygropyrexia Syndrome group and Splenogastric Hygropyrexia Syndrome treatment group was obviously higher than the normal group. The VEGF expression of Splenogastric Hygropyrexia Syndrome treatment group was obviously lower by the treatment. The TNF-αexpression of Splenogastric Hygropyrexia Syndrome group and Splenogastric Hygropyrexia Syndrome treatment group was obviously higher than the normal group before the treat. The TNF-αexpression of Splenogastric Hygropyrexia Syndrome treatment group was obviously lower in the post-treatment than it in the pretherapy.VEGF expression and thickness of coated tongue, ulcer ache and dropsy, difficult and heavy of limbs, course of disease have positive correlatious relationship. TNF-αexpression and thickness of coated tongue, ulcer ache and dropsy, ulcer exudation, difficult and heavy of limbs, course of disease have positive correlatious relationship.The body of the tongue and the coated tongue of the Splenogastric Hygropyrexia Syndrome patients of ROU which were treated by the KouKuiFang are obviously improved. Differences have highly significant statistical signifinance. The ulcer diameter, number, synathresis, dropsy, ache degree and healing time have all taken a turn for the better. The ulcer exudation information also has invariably extent amendment. With regard to the general symptom of patients such as difficult and heavy limbs, stool character and appetite. We find only difficult and heavy limbs better after the trtreatment.After the patients of Splenogastric Hygropyrexia Syndrome ROU were treated by the KouKuiFang, the clinical total effective rate can achieve 80 percent.3. Animal ExperimentObjects:SD SPF rats were adopted. The number of female and male rats is egual. It was randomly divided into 7 groups. Such as normal control group, dental ulcer group, dental ulcer Splenogastric Hygropyrexia Syndrome group, Splenogastric Hygropyrexia Syndrome group, dental ulcer cure group, dental ulcer Splenogastric Hygropyrexia Syndrome low dose group, dental ulcer Splenogastric Hygropyrexia Syndrome high dose group. The sample number of each group was 10.Method:For studying reproduction of animal model, we adopt the method of integrate of disease and syndrome. The dental ulcer model, dental ulcer Splenogastric Hygropyrexia Syndrome model, dental ulcer cure model, dental ulcer Splenogastric Hygropyrexia Syndrome cure model were manufactured by mean of NaOH crystal, honey, fats, the artificial climate cabin and KouKuiFang.Results:The dental ulcer Splenogastric Hygropyrexia Syndrome rats model appeared the following symptom, for example weight lose, hair loose rough, dysphoria, scrotum slack ptosis, not-forming s,tool or loose stool, anorexia, few drink, oral lip moisture, flowing saliva and so on.. KouKuiFang can improve above-mentioned symptoms after applying it.Mucosal ulceration, inflammatory exudate on theulcersurface, infiltration of many inflammatory cell, more necrotic tissue surrounding collagenoblast and new life blood capillary were observed under the microscope.VEGF and TNF-αexpression:The TNF-αexpression of dental ulcer group, dental ulcer Splenogastric Hygropyrexia Syndrome group, Splenogastric Hygropyrexia Syndrome group are obviously higher than the normal group before the therapy, and the TNF-αexpression is the most highest for the dental ulcer Splenogastric Hygropyrexia Syndrome group. But the TNF-αexpression is obviously lower after the therapy. The VEGF expression of dental ulcer group and dental ulcer Splenogastric Hygropyrexia Syndrome group are higher than the normal group and Splenogastric Hygropyrexia Syndrome group before the therapy, but the VEGF expression is obviously lower after the therapy.4. ConclusionsThe VEGF expression of ROD Splenogastric Hygropyrexia Syndrome patients are higher before the therapy. It hints that VEGF is a protection factor which can promote the ulcer concrete.The TNF-αexpression of ROU Splenogastric Hygropyrexia Syndrome patients are higher before the therapy. It hints that cellular immune functional disorder participates the invasion of ROU and is one of patho-physiological foundation of ROU Splenogastric Hygropyrexia Syndrome.The VEGF expression of ROU Splenogastric Hygropyrexia Syndrome patients and the local symptom of ulcer show positive correlation. This explains that VEGF as a protectional factor for the ulcer reparation shows higher when the clinical symptom is aggravated.The TNF-αexpression of ROU Splenogastric Hygropyrexia Syndrome patients and the local symptom of ulcer show positive correlation. This explains that the degree of immune functional disorder relates to the local pathological change. The TNF-αexpression can indirectly react the light and heavy press of the clinical symptom.The TNF-αand VEGF expression show lower after the therapy by KouKuiFang. This explains that KouKuiFang can improve the clinical symptom of patients, accelerate the concrescence of oral ulcer and has clinical curative effect precisely.Making rat model by the method of integrate of disease and syndrome appear near characteristic with the clinical splenogastric hygropyrexia syndrome. Measured syndrome by medicine may restore or improve many unusual experimental index. This confirmed that duplicating splenogastric hygropyrexia rat model accorded with the traditional Chinese medicine splenogastric hygropyrexia syndrome attribute.The symptom, pathology, VEGF and TNF-αchange in the duplicated rat dental ulcer splenogastric hygropyrexia syndrome model are consistent with the clinical ROU splenogastric hygropyrexia syndrome change. KouKuiFang may improve the above indexes, which indicated the curative effect of KouKuiFang is exact.
