Reviews On 116 Cases Of Liver Cirrhosis And The Effects Of JianPiYiGan Decoction On AQP8 Of The Experimental Liver Cirrhosis Rats

Purpose:The study contains two parts,the clinical reviewing study and the experimental study on liver cirrhosis.The study not only summarizes the clinical characteristics,the symptoms,the diagnosis types of TCM and the regulations of the Traditional Chinese Medicine(TCM)in treating liver cirrhosis(LC),but also observes if JianPiYiGan decoction can improve the expression of the AQP8 of liver to experimental rats through TCM treatment. In combining the experimental and clinical results,we would go one-step forward to expound the meaning of treating LC by JianPiYiGan decoction,in order to explore the new target for treating such disease,and also benefit the use of the clinical medicine practice.Method:1.The Reviewing StudyThe reviewing database cases consist of ll6 cases of LC from the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine' s medical record library.All the relevant cases will be included in the research according with the diagnostic standard.Of 225 investigated cases,there are 187 cases of man,and 37 cases of woman.According with the thesis diagnostic standard,116 cases are included in the study,96 cases of man and 20 cases of woman.2.Animal experiment:The mould is based on Yang Ting's experiment study.The animal experiment consists of ninety healthy SPF-grade Wistar rats, there were ten rats for the blank group(group A)which was picked randomly from them.Of the left rats(80 rats)are going on the mould make of the LC. On the first day of experiment,the eighty mould rats were injected with CCL4 hypodermically,5 mini-liter per kilogram(mL/kg)of the rats' weight,except blank group.Then,they are hypodermically injected CCL4-olive oil solution, in the ratio of four-to-six(4:6),with the dosage of 3mL/Kg,twice per week lasting for two weeks.The weights of the rats were examined once every week in order to adjust dosages of the CCL4-live oil solution.They were fed with ordinary fodder with freedom.Group A was hypodermically injected with the needed dosage of olive oil.At the end of the twelfth week,there were fifty-four mould rats left,randomly picked eight rats for pathological use.The other rats are divided into five groups at random treating them JianPiYiGan decoction with high,medium and low dosages,observing the curative effects on the groups of JianPiYiGan decoction and the contrasting group.Results:1.Reviewing study shows The age distributions of the LC patients' are as followings:the peak age range of this disease is at the age of fifty to sixty years old;this reflected the age characteristic of the LC.The in-hospital days are between twenty-one to thirty days at its most;this explains the repeated and obstinate trend of the liver cirrhosis.The typical five-zang viscera syndrome,the spleen systematic symptoms ranks for the first place,hepatic systematic symptoms counts for the second,and then the cardiac system,the pneumonic system and renal system.These obviously indicate that the symptoms of the LC patients' are the spleen and liver systems.Of all disease causes,by means of TCM, "dampness" ranks up for the major factor of causing such disease(LC),the viscera that involved the most are spleen and liver.Of all types,"retention of damp pathogen" ranks for the first place,"accumulation of dampness and heat pathogen" takes the second and "stagnation of liver-qi" follows as the third.Herbs(TCM)that use the most in treating LC are the ones with the ability to "invigorating the spleen and dispersing the dampness",and then with the ability of smoothening and regulating the flow of vital energy of the liver.The herbs that use the most include the main factors of the ancient prescription "WuLingSan." This shows that invigorating the spleen and dispersing the dampness occupies a very important position in treating hepatic disease in TCM.Therefore,the results embody the characteristics of "the Correlation of Liver and Spleen of TCM," and dampness is closely relevant with this disease.And the clinical therapies are also surrounding tightly with "invigorating the spleen and dispersing the dampness" in treating this kind of hepatic disease.2.The experimental results:The results of RT-PCR and Western Blotting show,the AQP8 quantity of blank group has remarkable differences with the model group(P<0.01).The marks of AQP8 rise tremendously after treating by JianPiYiGan decoction,the curative effects are among them in positive correlation with the dosage use of the TCM.There are comparable differences between Group E and group B (P<0.05),and there are extremely remarkable differences between Group C and B,group D and B(P<0.01).Therefore,the therapeutic results that arises the quantity of AQP8 by JianPiYiGan decoction could be the new target of treating LC.Conclusion:The thesis concentrates on the clinical curative effects of the liver cirrhosis through systematic retrospect of the clinical reviewing study and the experimental study on the liver cirrhosis.The experiment used modern molecular biological technology,such as RT-PCR and Western Blotting,to find out genetic expression and protein expressions of rats' AQP8 of their liver, and reach the following results:1.The age distribution of liver cirrhosis:the peak period of having LC is at the ages between 51-60 years old,and the age range between 61-70 years old takes the second place.But this might related to the causes of death.2.The frequency of liver cirrhosis symptoms shows that the spleen system's symptom has the highest proportion of all,hepatic system follows. This indicated that,in correlation of five-zang viscera of TCM theory,spleen and liver is in close relations.3.The diagnose types that related to "dampness" count for 34.5%,and the types that related to blood stasis count for 27.6%.This embodies that dampness is the major contradiction of treating LC,and blood stasis comes next.4.Highly frequent used herbs in treating LC are Poria(fu ling),Rhizoma Atractylodis Macrocephalae(bai zhu),Radix Glycyrrhizae(gan cao),Rsdix Salvia Miltiorrhizae(dan shen),Rhizoma ilismatis(ze xie),Polyporus umbellatus(zhu ling).Four of them are the major bones of the "WuLingSan" which is an well-known ancient prescription for treating symptoms of water retention.This show that reinforcing the spleen with dispersing the dampness method occupies the most percentage,the method of "promoting blood circulation and dispelling stasis" comes the second,the method of "smoothening and regulating the flow of vital energy of the liver" follows as the third.Therefore,the results embody the characteristics of "the Correlation of Liver and Spleen of TCM," and dampness is closely relevant with this disease.And the clinical therapies are also surrounding tightly with "invigorating the spleen and dispersing the dampness" in treating this kind of hepatic disease.5.As the reviewing study shown,LC patients' diagnosis types,using frequency of herbs,the behavior of five-zang viscera are tightly joined with spleen and hepatic systems of TCM.And the use of herbs is mainly by means of adjusting spleen and hepatic systems of TCM in order to treating LC functionally.6.RT-PCR results show that JianPiYiGan decoction can improve the expression of AQP8 of rats' liver on the level of mRNA.7.The results of Western Blotting verify that JianPiYiGan decoction can improve the expression of AQP8 of LC rats' liver on the level of its protein.8.Animal experiment shows that traditional Chinese medicine can obviously improve the expression of the liver cirrhosis rats' AQP8.This evidence is not only one of its molecular biological foundations of invigorating the spleen and dispersing the dampness,but also offer a strong laboratory basis for the Correlation of Liver and Spleen of TCM in treating LC.
