Objective:(1)To investigate the clinical effect on reflux esopHagitis of invigorating the spleen,dispersing the stagnated liver-qi, activating qi flow and calming the adverse rising qi decoction. (2)To study the mechanism of the treatment rules of "invigorating the spleen, dispersing the stagnated liver-qi, activating qi flow and calming the adverse rising qi" on RE by animal experiment in the respect of accommodating gastroesopHageal motivation.Method:(1)Clinic research:60 reflux esopHagitis patients with TCM syndrome of stagnation of liver qi and spleen deficiency were divided into two groups randomly:treatment group are cured by Chinese Medicine and control group by western medicine. The two groups were respectively given invigorating the spleen,dispersing the stagnated liver-qi, activating qi flow and calming the adverse rising qi decoction and Mosapride Citrate Tablets for 12 weeks. We study the change of major and secondary symptoms before and after the treatment.(2)Experiment study:â‘ The rat models of reflux esopHagitis were established by "partly pyloric ligation+cardiomyotomy". We randomly divided them into seven groups:blank controlled group, model group, placebo group, western medical controlled group, and high,middle and low dosage of Chinese medicine groups. The blank controlled group and the model group was given normal saline, while the other groups were respectively given Oryzanol Tablets, Mosapride Citrate Tablets, high,middle and low dosage of Chinese medicine by gastroperfusion. Treatment lasted for 3 weeks. After 3 weeks, we study the effect of the treatment by observing pH of lower esopHagus, improvement of esopHageal mucous membrane, and pathological changes under microscope.â‘¡Making use of Power Lab Multi-functional pHysiological & pHarmacological recorder, synchronous recording the grapH of activity changes about the esopHageal and stomach fundus smooth muscle strips of isolated rat.After stabled, the muscle strips were respectively given normal saline, high,middle and low dosage of Chinese medicine, and Mosapride Citrate Tablets, recording the 5 minutes' contraction grapH before and after. To observe the effect on the muscle strips, which were respectively given high,middle and low dosage of Chinese medicine and Mosapride Citrate Tablets, and normal saline group was selected as control group.We study the change of the effect on the esopHageal and stomach fundus smooth muscle strips, after respectively giving above medicine.Result:(1)Clinic research:The two clinic groups all could relieve the major and secondary symptoms, and had no significant difference between them(P>0.05). Treatment group being cured by Chinese Medicine had significant change of some clinic symptoms, such as frequency of chest pain, belch, abnormal bowel movement, chest pain, irritability compared with control group.And the recurrence ratio of treatment group is superior to control group(P<0.05).The two groups all could improve the manisfestations under microscope,and the improvement of treatment group was better than control group(P<0.05).(2)Experiment study:â‘ Invigorating the spleen, dispersing the stagnated liver-qi, activating qi flow and calming the adverse rising qi decoction could improve pH of lower esopHagus, alleviate pathology injure.â‘¡Invigorating the spleen, dispersing the stagnated liver-qi, activating qi flow and calming the adverse rising qi decoction could strengthen the esopHageal and stomach fundus smooth muscle's amplitude of contraction of isolated rat.And the effect on stomach fundus smooth muscle was intensified compared with that of the control group.Conclusion:invigorating the spleen, dispersing the stagnated liver-qi, activating qi flow and calming the adverse rising qi decoction has effect on RE. We infer the mechanism is improve motivation, regulate inflammatory factors of mucous membrane of esopHagus, etc.
