Objective: To observe the effect of KangNaoLing capsule on the patients with vascular cognitive impairment no dementia(VCIND) and explore the mechanism of it. Methods: Compare with Piracetam,observe the influence of KangNaoLing on the cognitive abilities, clinical symptom, mean cerebrovascular blood flow velocity, hemorheology, blood fat, event-related potential P300 and vascular motor reactivity(VMR)of patients suffered from VCIND and check its security at the same time. Also make experiment on rats to determine the influence on the ability to learn and memory, the neuron appearance of the hippocampus, expression of synaptophysin(Syn), rCBF and long-term potentiation(LTP)of the rats model with vascular cognitive impairment by 4VO methods. Results: After the treatment of KangNaoLing capsule, the total clinical efficiency is 61.40﹪,neuropsychological examinations such as MMSE, clock-drawing task(CDT), aletter cancel tes(tLCT) and continuous calculation tes(tCCT)show obvious amelioration of the sufferings'cognitive ability. The total efficiency of TCM syndrome therapeutic evaluation is 82.46﹪,the clinical syndrome ameliorated clearly. The latent period of event-related potential P300 decurtated obviously and the wave amplitude run up clearly. VMR raised obviously. The index of hemorheology, the cerebrovascular mean blood flow velocity and blood fat also has apparent improvement compared with the states before treatment. There's little adverse effect in the treatment progress. The security evaluation is good. In experimented study, the achievement of the testees(rats took KangNaoLing capsule) are more good than the contrasts(rats took Piracetam) and models(rats took nothing) on place test and spatial probe test. The neuron appearance of the testees'hippocampus is regular, there's no clear appearance of ischemia and necrosis. Compare with contrasts and models, there's more expression of Syn. The rCBF of hippocampus increases apparently. The LTP in CA1 area of hippocampal slices of rats isincreased evidently. Conclusions: It was concluded that KangNaoLing capsule significantly improve the cognitve functions of patients with VCIND and VCI rats. The relevant mechanism may be related to expanding cerebrovascular, improving cerebral blood circulation, enhancing cerebrovascular reserve capacity, depressing blood viscosity, amending hemorheology, accommodating blood fat, accommodating neuron synaptic plasticity and enhancing synapse's quantity. Activating blood circulation to dissipate blood, invigorating the kidney and KangNaoLing capsule is a valid therapy to VCIND.
