In order to mitigate the risk of the burden of care expenses caused by disability and dementia,the first batch of pilot programs for long-term care insurance began in 2016 and the second round of pilot programs began in 2020.However,the current pilot work is not perfect,the areas involved are still very limited.Of the more than 600 cities in the country,only 49,including Changchun,Chengdu and Qiqihar,have carried out the pilot program.Therefore,this study intends to explore the development of long-term care system in order to effectively solve the problem of long-term care of the disabled elderly and further consolidate its achievements.This study used panel data from the China Health and Pension Tracking Survey(CHARLS)2015 and 2018 to analyze the first pilot cities under the DID dual difference framework,and the results showed that the pilot program of long-term care insurance policy was effective.Based on the empirical analysis of the long-term nursing insurance system in our country,it is found that the financing structure is not perfect,the service orientation is not perfect,and the legal system is weak.Finally,some suggestions were put forward to optimize the pilot policy of long-term care insurance.The main conclusions are as follows: the implementation of long-term care insurance pilot policy is significant.(1)The medical expenses have been saved,and the monthly outpatient expenses for the disabled elderly in the pilot areas have been reduced by 0.494%,the number of monthly outpatient services by 0.249,the annual hospitalization expenses by 0.55% and the number of annual hospitalization by 0.264.The number of chronic diseases of the disabled elderly decreased by 0.634.By changing the nursing structure and improving the availability of nursing services,the probability of being cared for disabled elderly increased by 70.8%,and the probability of receiving formal care increased by73.6%.(2)In terms of the objects of protection,the coverage is limited,mainly including urban workers’ insurance groups,and there is a lack of attention to rural residents.In the scope of security,mostly long-term medical care services,some areas do not provide day-to-day care and nursing services.In the fund-raising channels,although emphasizing the individual and unit fund-raising responsibility,but still mainly rely on the overall fund transfer to raise funds,unsustainable.In terms of financing standards,there is a great disparity in the quota financing standards among different regions,and the payment proportion is far lower than the international level.In nursing services,the standards of nursing grading are not clear,and it is difficult to match the nursing needs of disabled people with medical care resources.In terms of nursing resources,there is a big gap between urban and rural nursing resources,and it is difficult to meet the daily care needs of disabled elderly in rural areas.In the construction of the legal system,the relevant laws and regulations are not perfect,the policy is fragmented is not systematic.This paper provides an important theoretical basis for further promoting the pilot policy of long-term care insurance and finally promoting it nationwide.
