Research On Social Work Intervention In The Combination Of Medical Care For The Aged: Taking On The Y Pension Institution In Nanjing As An Example

Since the 21 st century,China’s aging has accelerated,and the situation and problems of the elderly have become increasingly serious.The absolute population of the elderly is relatively large,and the number of elderly people with advanced age,disease,disability(semi-disability)and dementia is significantly on the rise.Their dual needs for medical and health services and basic living care are increasingly prominent.Since 2013,China has begun to develop the "combination of medical care and elderly care" model,actively promote the close combination of medical care and elderly care services,explore a new model of elderly care,and put forward a new way to alleviate the aging problem.Traditional pension institutions focus on meeting the physiological needs of the elderly,while the combination of medical care and nursing mode simultaneously meets the medical and rehabilitation needs of the elderly,while the intervention of social work can more effectively meet the psychological and spiritual needs of the elderly.In this thesis,Y Medical and elderly care combined institution in Nanjing was taken as the research object.and the development status of medical and elderly care combined institution was understood by collecting and searching relevant literature and materials.And through the questionnaire method,interview method,observation method to the elderly,management staff and social workers in the institution of investigation and interview.The results show that the elderly in the current combination of medical and nursing institutions need medical care,cultural entertainment,spiritual comfort and social communication.Through a series of professional activities,such as case work and group work,social workers in the institutions provide spiritual care and emotional support for the elderly,which cannot be provided by traditional institutions.After research showed that social work intervention Y d a combination with the effects of endowment institutions,mainly reflected in the following four aspects: the elderly higher satisfaction for social workers and social work services,agency service level improved,departure from the hospital rate lower,to establish a set of professional social work services mode,integrate social resources,develop new service projects.Social workers in institutions have problems of social belonging,low sense of identity and high staff turnover.However,for the social work involved in the combination of medical and nursing institutions for the elderly,there are still some problems,such as the imperfect development of government system,the imperfect development of social work service standard system and evaluation system,the insufficient practical experience of social workers,the relatively single service content,and the marginal position.In the face of all these problems,we will further establish and perfect the social work involved in medical nursing services policies,giving medical professionals endowment institutions to introduce capital support,build system of social work business practices and evaluation mechanism,strengthen medical raise pension institutions to foster social workers provide more specific countermeasures and Suggestions.
