Purpose:The aim of this study is to observe the clinical efficacy of the Bletilla striata and Panax notoginseng combined with esomeprazole magnesium enteric-coated tablets in the treatment of artificial ulcer after Endoscopic submucosal dissection(ESD)of esophageal lesions, and to explore the safety and efficacy of the traditional Chinese medicine combined with esomeprazole magnesium enteric-coated tablets in the treatment of esophageal ulcer.Material and method : This study was a single-center prospective randomized controlled trial.A total of 48 subjects who received ESD treatment of esophagus were enrolled and randomly divided into experimental group and control group,24 cases in each group. The experimental group was treated with traditional Chinese medicine Baihe,Panax notoginseng combined with esomeprazole magnesium enteric-coated tablets, and the control group was treated with esomeprazole magnesium enteric-coated tablets alone, and the treatment period was 4 weeks. After 4 weeks of treatment, the TCM syndromes and ulcer healing status of the two groups were evaluated.Results:1.A total of 48 patients who agreed to participate and completed data collection were included in this study,24 in the experimental group and 24 in the control group. In terms of general data, there were no significant differences in gender, age, related medical history, lesion location, postoperative diagnosis, operation time, hospitalization days, and hospitalization expenses between the two groups. In addition,en bloc resection(48/48)and curative resection(45/48)were achieved in both groups.Complications included intraoperative perforation in 2cases(4.17%),infection in 18 cases(37.50%),bleeding in 2 cases(4.17%),and postoperative stenosis in 3 cases(6.25%).2.At 4 weeks after operation, the total TCM syndrome scores of the two groups decreased significantly(P<0.001),and the experimental group decreased significantly(P=0.004),with statistical difference. In addition, the total efficacy of the experimental group was significantly better than that of the control group at 4 weeks after operation, with statistical difference(P=0.006).3.There was no significant difference in the initial ulcer area between the two groups after surgery. At 4 weeks after surgery, the ulcer area of the two groups was reduced, and the experimental group was reduced more significantly than the control group(P=0.046).In addition, the ulcer reduction rate of the experimental group was 100%(97.92%-100%),and the control group was 96.50%(81.25%-100%).The difference was statistically significant(P=0.009).4.In terms of medication safety, the liver function and renal function indexes(ALT,AST,CREA,UREA)of the two groups were in the normal range at 4 weeks after operation, and no serious adverse drug reactions occurred.Conclusion:1.In the treatment of artificial ulcer after ESD,the improvement of TCM symptoms in the combination group is better than the single esomeprazole magnesium enteric-coated tablets group.2.In the treatment of artificial ulcer after ESD,the healing rate of ulcer and the reduction rate of ulcer in the combination group are better than those in the single esomeprazole group.
