Purpose:This study aims to explore the efficacy and safety of Western medicine combined with Linggui Zhugan Tang in the treatment of liver cirrhosis ascites with spleen and kidney yang deficiency through clinical observation,providing new ideas and methods for the treatment of liver cirrhosis ascites.Material and method:This study is a retrospective analysis,collecting and sorting out the cases of 92 patients with cirrhosis ascites from September 2020 to September 2022,and the syndrome differentiation of traditional Chinese medicine is spleen and kidney yang deficiency,and regular re-diagnosis,45 patients are treated with integrated traditional Chinese and western medicine as the treatment group,47 patients are treated with western medicine as the control group.The control group received etiological treatment and appropriate salt restriction treatment according to the scheme recommended in the guidelines of western medicine.On the basis of western medicine treatment,the treatment group was given Linggui Zhugan Decoction twice a day.Both groups were treated for one month.Observe the TCM symptoms,clinical symptoms,liver and kidney function,abdominal circumference,body weight,24-hour average urine volume,ascites degree and other indicators of the two groups before and after treatment,and finally use SPSS25.0 statistical software for statistical analysis.Results:1.Inclusion of cases: 92 cases(45 cases in the treatment group and 47 cases in the control group)completed the treatment and follow-up for 1 month.2.Comparison of comprehensive efficacy: the total effective rate of the treatment group was88.89%,and that of the control group was 63.83%.The total effective rate of the treatment group was better than that of the control group(p<0.05).3.Comparison of TCM syndrome score: Before and after treatment,the data of each group were compared within the group,and the difference was statistically significant(p<0.05).After treatment,the improvement effect of TCM syndrome in the treatment group was better than that in the treatment group(p<0.05).4.Comparison of curative effect of TCM syndromes: the total effective rate of the treatment group was 91.11%,and the total effective rate of the control group was 65.96%,indicating that the treatment group had better curative effect(p<0.05).5.Comparison of liver function: ALT,AST,ALB,TBIL and CHEin each group were compared before and after treatment,all p<0.05,the difference was statistically significant.After treatment,ALT,AST,TBIL and CHE in the treatment group were significantly improved compared with those in the control group(p<0.05);There was no significant difference between ALB groups(p>0.05).6.Comparison of coagulation function: PT and PTA in each group were compared before and after treatment(p<0.05),and the difference was statistically significant.After treatment,the blood coagulation function of the treatment group was significantly improved compared with the control group(p<0.05).7.Abdominal ultrasound comparison: The spleen thickness,ascites depth and ascites grading of each group were compared before and after treatment,and the above indexes in each group were significantly improved(p<0.05),with statistical significance.The comparison between the groups after treatment showed that the improvement of ascites depth and ascites grading in the treatment group was better than that in the control group(p<0.05);There was no significant difference in spleen thickness between the two groups(p>0.05).8.Comparison of average 24-hour urine volume,abdominal circumference and body weight:The average 24-hour urine volume,abdominal circumference and body weight of each group before and after treatment were compared within the group,all p<0.05,the difference was statistically significant.The comparison between the groups after treatment showed that the average 24-hour urine volume and abdominal circumference of the treatment group were significantly different from those of the control group(p<0.05);There was no significant difference between the weight groups(p>0.05).9.All patients have no obvious side effects or adverse reactions during the treatment.This study is safe and feasible.Conclusion:The combination of modified Linggui Zhugan Tang and Western medicine treatment can significantly improve the symptoms,signs,and main physicochemical indicators of patients with liver cirrhosis ascites due to spleen and kidney yang deficiency.It can significantly improve the prognosis of patients and has good clinical value.
