Objective: To discuss the application of video head impulse test in differentiating vestibular migraine and posterior circulation ischemia caused dizziness. To investigate the application of video head impulse test(VHIT) in differentiating vertigo caused by vestibular migraine and posterior circulation ischemia.Methods: From January 2015 to August 2015, enrolled 60 patients with vestibular migraine as the vestibular migraine group and 50 patients with posterior circulation ischemia as the posterior circulation ischemia group contained, otherwise, selected 60 cases as control group in the health physical examination at the same time from the Xinjiang medical university first affiliated hospital, VHIT and caloric test were undertaken the subjects.Results: Among patients of VM group, the gain value of vestibular-ocular reflex(VOR)slow phase on the lesion side was lower than that on the normal side and the positive rate of compensatory saccade on the lesion side was higher than that on the normal side( P<0.05).Among patients of PCI group, the gain value of(VOR) slow phase on the lesion side was not significantly different from that on the normal side(P>0.05); the positive rate of compensate or saccade of the lesion side was higher than that of the normal side( P<0.05). The three groups were significantly different in the gain value of VOR slow phase and the positive rate of compensatory saccade( P<0.05); VM group was lower than control group and PCI group in the gain value of VOR slow phase(P<0.01);VM group was higher than control group and PCI group in the positive rate of compensatory saccade( P<0.05). the positive rates of VM were 76.7%(46/60) in VHIT,48.3%(29/60) in caloric test. Significant difference(P<0.05) was observed between VHIT and caloric test examined by chis-quaretest. There was statistically significant difference in the positive rate of caloric test among the three groups(P<0.05).Conclusion: VHIT can be used to differentiate the vertigo caused by VM and PCI.Compared to caloric test, the rate of abnormal video head impulse test in the vestibular migraine group was significantly higher than that caloric test.
