Effect Of Shen'an Decoction On Serum VEGF And Urine KIM-1 In Patients With Diabetic Nephropathy With Spleen And Kidney Yang Deficiency Syndrom

Objective:In this paper,through modern clinical testing methods,to observe the effect of endothelial growth factor(VEGF)and kidney injury molecule-1(KIM-1)of Shenan Decoction on the diabetic kidney disease with spleen-kidney Yang deficiency.The mechanism and safety of Shenan Decoction were explored to provide objective basis for the treatment of diabetic nephropathy with traditional Chinese medicine.methods:According to the inclusion criteria and exclusion criteria,60 cases of diabetic nephropathy with spleen-kidney Yang deficiency syndrome were randomly divided into observation group and control group,with 30 cases each group.Both groups were required to receive conventional Western medicine treatment.30 cases in observation group were added Shenan Decoction on the basis of conventional Western medicine treatment,and 2 weeks were set as a course of treatment.The changes of symptoms,fasting blood glucose,2-hour postprandial blood glucose,glycosylated hemoglobin,blood lipid,serum creatinine,urea nitrogen,24-hour urinary protein quantity(infection and female menstrual period should be excluded when retained),serum ascular endothelial growth factor,urinary kidney injury molecule-1 and other biochemical indexes were compared and observed.All therapeutic indexes were measured before and after treatment.Results:1.After treatment,symptoms of both groups were improved(P<0.05),and the improvement in the observation group was significantly better than that in the control group(P<0.05);2.After treatment,the fasting blood glucose,2 hours postmeal blood glucose,blood lipid,serum creatinine,urea nitrogen and other biochemical indexes of the two groups were improved compared with those before treatment(P<0.05),of which the observation group was more effective than those of the control group(P<0.05),suggesting that Shenan decoction was effective in the treatment of diabetic nephropathy,and better than conventional Western basic treatment.There was no significant change in HBAlc between the two groups before and after treatment(P>0.05).3.After treatment,the 24-hour urinary protein quantity in both groups were decreased compared with that before treatment(P<0.05),and the observation group was significantly better than the control group(P<0.05),suggesting that Shenan decoction can effectively reduce urinary Protein on base of conventional Western medicine basic treatment;4.After treatment,VEGF and KIM-1 indexes were decreased in both groups(P<0.05),and observation group was better than control group(P<0.05),suggesting that Shenan decoction can reduce the degree of inflammatory response indiabetic nephropathy patients.Conclusion:1.In addition to conventional Western medicine treatment,Shenan Decoction can be used to treat diabetic nephropathy with spleen-kidney Yang deficiency syndrome.Compared with simple conventional Western medicine basic treatment,Shenan Decoction can better improve the symptoms of exhaustion and cold,swollen limbs,waist cold and knee acid and urine turbidity of the patients,and has certain effects to improve blood sugar and blood lipid.Delay the progression of kidney damage and other complications.Its mechanism may be related to inhibiting inflammatory response.2.No obvious adverse reactions and toxic side effects were observed Shenan Decoction on diabetic nephropathy with spleen-kidney Yang deficiency,which is worthy of further promotion.
