| Purpose:To observe the clinical efficacy of ear point pressing bean method in the treatment of hyperemesis gravidarum with disharmony between liver and stomach and its effect on laboratory indexes such as urinary ketone body and electrolyte,and to explore its mechanism,so as to provide theoretical basis for clinical promotion.Material and method:62 patients with hyperemesis gravidarum with disharmony between liver and stomach were randomly divided into experimental group and control group,with 31 cases in each group.The experimental group was treated with ear acupoint pressing bean combined with Jiangni zhiou Decoction and Western medicine rehydration,and the control group was treated with Jiangni zhiou Decoction and Western medicine rehydration for7 consecutive days,and were followed up to 28 weeks of pregnancy.The changes of total effective rate,TCM syndrome score,urinary ketone body negative conversion rate,electrolyte(potassium ion and sodium ion),PUQE scale score and repeated hospitalization rate were compared between the two groups before and after treatment.Results:1.The differences in age,days of menopause,duration of illness and pre-treatment PUQE scale score,TCM evidence score,electrolyte levels(potassium and sodium)and urinary ketone bodies between the two groups before treatment were not statistically significant(P>0.05)and were comparable.2.Comparison of the total clinical effect after treatment: the total effective rate of the test group was 93.55% after treatment,which was higher than the total effective rate of the control group of 77.42%,the difference was statistically significant(P<0.05).3.Comparison of TCM evidence points before and after treatment: intra-group comparison:both treatments improved the patients’ TCM symptoms,with statistically significant differences(P<0.05);inter-group comparison after treatment: the test group showed significant improvement in reducing TCM clinical symptoms compared with the control group,with statistically significant differences(P<0.05).4.Comparison of the degree of nausea and vomiting before and after treatment: intra-group comparison: both treatments were able to reduce the degree of nausea and vomiting of the patients,the difference was statistically significant(P<0.05);inter-group comparison after treatment: the test group was more effective than the control group in reducing the degree of nausea and vomiting of the patients,the difference was statistically significant(P<0.05)5.Comparison of urinary ketone body levels before and after treatment: intra-group comparison: the conversion rate was 74.19% in the test group and 45.16% in the control group,both treatments improved urinary ketone body levels,the difference was statistically significant(P<0.05);inter-group comparison after treatment: the test group was better than the control group in improving urinary ketone body levels,the difference was statistically significant(P<0.05).6.Comparison of electrolytes before and after treatment: both treatments improved the patients’ blood potassium and sodium levels in an intra-group comparison,which was statistically significant(P<0.05);the difference between the two groups in terms of the improvement of blood potassium and sodium levels after treatment was not significant and not statistically significant(P>0.05).7.Comparison of repeat hospitalisation rates between the two groups: patients in both groups were followed up by telephone up to 28 weeks’ gestation,and when comparing repeat hospitalisation rates,patients in the treatment group had a significantly lower rate of repeat hospitalisation,with a statistically significant difference(p<0.05).8.No significant adverse effects occurred in either group.Conclusion:1.Compared with traditional Chinese medicine decoction combined with western medicine rehydration therapy,combined ear point pressing bean therapy can more effectively reduce the TCM syndrome score of patients with liver stomach disharmony and hyperemesis gravidarum,and reduce the clinical symptoms such as nausea and vomiting;2.Compared with traditional Chinese medicine decoction combined with western medicine rehydration,the combination of ear point pressing and bean therapy can accelerate the improvement of urinary ketone body level and electrolyte(potassium and sodium)level in patients with hyperemesis gravidarum with disharmony between liver and stomach;3.Compared with traditional Chinese medicine decoction combined with western medicine rehydration therapy,combined ear point pressing bean therapy can significantly reduce the repeated hospitalization rate of patients with hyperemesis gravidarum with disharmony between liver and stomach and reduce the incidence of long-term complications;4.Auricular bean pressing therapy has the advantages of small side effects,simple operation,safety and effectiveness. |
