Objective:To study the clinical effect of risperidone combined with repetitive transcranial magnetic stimulation(TMS)and its effect on the integrity of white matter in first episode schizophrenia.Methods:80 cases of first-episode schizophrenic patients who were treated in the hospital of Zaozhuang Mental Health Center for 2015.10 months~2018.10 months were selected as the subjects.By prospective analysis,the patients were randomly divided into 2 groups,and 40 patients were included in the control group and the observation group.In the control group,40 patients were treated with risperidone alone,and 40 of the patients in the observation group were treated with risperidone and repetitive transcranial magnetic stimulation.Two groups of patients in clinical treatment of concrete were observed with PANSS positive and WCST negative symptom scale and Wisconsin Card Sorting Test were used to evaluate differences before and 4 weeks after treatment in two groups were observed during treatment,two groups of patients the incidence of side effects,evaluation of two groups of patients before treatment,treatment 4 weeks Houbai matter anisotropy change difference by magnetic resonance imaging,and treatment satisfaction questionnaire.Results:Before treatment,there were no significant differences between the two groups were compared with PANSS score(P>0.05),was not statistically significant;after 4 weeks of treatment,the observation group PANSS scale positive factor score(12.1±1.6),negative factor score(11.9±1.3)points,general pathology score(23.5±1.9),total score(49.5±2.6),were significantly lower than the control group,compared with significant difference(P<0.05),with statistical significance.Before treatment,there were no significant differences between the two groups were compared with WCST score(P>0.05),was not statistically significant;after 4 weeks of treatment,the observation group WCST test project,complete classification number,the correct responses were(3.5 ± 0.2),(68.5±2.3),were significantly higher than control group the answer is wrong,(57.3±2.7),significantly lower than the control group,compared with significant difference(P<0.05),with statistical significance.Patients in the observation group during the treatment of all kinds of side effects,the total incidence rate was 15.00%(6/40),the control group was 12.50%(5/40),the comparison between the two groups had no significant difference(x2=3.115,P= 0.235),was not statistically significant.Before treatment,two groups of patients with white matter fractional anisotropy comparison showed no significant difference(P>0.05),was not statistically significant;after 4 weeks of treatment,the observation group of patients with left anterior cingulate gyrus and left posterior cingulate white matter anisotropic fraction changes respectively(0.23±0.03),(0.29±0.03),were significantly higher than control group,compared with significant difference(P<0.05),with statistical significance.The observation group of patients treatment the total satisfaction rate was 95.00%(38/40),significantly higher than the control group 77.50%(31/60),the difference between the two groups was significant(x2=9.338,P= 0.002),with statistical significance.Conclusion:risperidone combined with repetitive transcranial magnetic stimulation in the treatment of first-episode schizophrenia effect,can play a great role in the short term,has a positive effect on improving the patients with mental symptoms,and can delay or prevent the patients with left anterior cingulate gyrus and left posterior cingulate white matter integrity damage to further consolidate the curative effect.This method can be used as the basic treatment for first episode schizophrenic patients.
