Objective To investigate the effect of spousal follicular fluid on sperm quality by incubating spermatozoa of patients with asthenospermia,to determine the changes of sperm quality and sperm function at different times,to clarify the mechanisms of follicular fluid on sperm function and oxidative stress damage,and to provide the scientific basis for improving the fertilization rate and clinical pregnancy rate of IVF/ICSI-ET.Methods Sixty patients with moder ateasthenospermia who underwent IVF/ICSI-ET in Reproductive Medicine Center,General Hospital of Ningxia Medical University from from October 2021 to January 2023 were collected.Follicular fluid from both ovaries was collected and marked on the OPU according to bilateral ovaries.The sperm incubated with the follicular fluid(divided into left and right side groups)were set up the study group,the control group used G-IVF and the blank group used NS.The changes of different times of total sperm motility,progress motility,sperm survival rate,sperm deformity rate,sperm DNA fragmentation index,sperm acrosome reaction rate and sperm reactive oxygen species were compared between the three groups.Density the sperm DNA fragmentation rate,sperm acrosome reaction rate and sperm reactive oxygen species with Flow cytometer.SPSS 23.0 software was used for statistics,and the measurement data were expressed by mean±standard deviation(x ± s)repeated measures analysis of variance.Independent sample t test was used for comparison between the two groups.P<0.05 was statistically significant.Results(1)The total sperm motility,percentage of PR sperm and sperm survival rate of the study group were significantly higher than those of the control group and the blank group after two hours and four hours of incubation,and higher than those before(P<0.01).(2)The total sperm motility,percentage of PR sperm and sperm survival rate of the study group at two hours were significantly higher than those in the group at four hors and eight hours(P<0.05,P<0.01),with indicators gradually decreasing with incubation time and the highest values of each parameter at 2 hours.(3)The sperm acrosome reaction rate of the study group was significantly higher than those of the control group and the blank group at two hours(59.16±9.87% vs.56.97±9.68% vs.54.81±13.92%,P<0.05),while the sperm DNA fragmentation rate and sperm reactive oxygen species were significantly lower than those of the control and blank groups(P<0.05).(4)The total sperm motility,percentage of PR sperm and sperm survival rate of the right follicular fluid group were higher than those of the left(all P<0.05).Conclusion(1)Incubating spermatozoa of patients with asthenospermia with follicular fluid significantly improves sperm quality and sperm function.(2)The best priority time of follicular fluid on sperm in vitro is two hours.(3)Follicular fluid is superior to G-IVF and is expected to be used as a physiological sperm activator for ART.(4)The right follicular fluid has a significantly higher promotion effect on sperm quality and sperm function than the left,and bilateral ovaries funtion exist physiological differences.
