A Preliminary Study On The Relationship Between Sperm Reactive Oxygen Species And Sperm Quality In Infertile Smoking Male

Abstract/Summary:
Objective:Study the effects of smoking on sperm reactive oxygen species(ROS)and sperm quality in infertile male with different degrees of smoking,and explore the possible mechanism of sperm quality decline caused by smoking by comparing the parameters of male spermatozoa.Therefore,it can provide clinical guidance for infertile male to solve infertility problems.Methods:The semen samples of infertile male treated in the Andrology Department of Northern Jiangsu People’s Hospital from December 2019 to February 2021 were retrospectively analyzed.116 infertile male were included according to the inclusion and exclusion criteria.According to the degree of smoking,116 infertile male were divided into Group A(non-smoking group,never smoking or quit smoking for over 6 months):41 cases,Group B(mild smoking group,smoking quantity ≤5 pack-years): 37 cases.Group C(moderate to heavy smoking group,smoking quantity >5 pack-years): 38 cases.Routine semen parameters,sperm ROS,sperm mitochondrial membrane potential(MMP)and sperm DNA fragmentation index(DFI)were detected in all semen samples.The results were analyzed by SPSS26.0,the difference among groups were analyzed by OneWay ANOVA,and the correlation between groups was analyzed by Person Test.Results:(1)There was no significant difference in age,body mass index(BMI)and days of abstinence among groups A,B and C(P>0.05).(2)There was no significant difference in semen volume,sperm concentration,sperm normal morphology rate and sperm MMP between group B and group A(P>0.05),but the sperm forward motility rate decreased significantly in group B.Besides,compared with group A,sperm ROS increased significantly(P<0.05)and sperm DFI increased significantly(P<0.01)in group B.(3)There was no significant difference in semen volume between group C and group A(P > 0.05),but compared with group A,the sperm concentration decreased significantly in group C(P<0.05)and the sperm ROS and sperm DFI increased significantly in group C(P<0.01).In addition,the sperm forward motility rate,sperm normal morphology rate and sperm MMP decreased significantly in group C(P<0.01).(4)There was no significant difference in semen volume,sperm concentration,sperm forward motility rate,normal sperm morphology rate,sperm MMP and sperm DFI between group C and group B(P>0.05),but compared with group B,sperm ROS increased significantly in group C(P<0.01).(5)Through Person test,it was found that there was a significant positive correlation between smoking quantity and sperm ROS(r=0.317,P<0.01).ROS in sperm was negatively correlated with sperm concentration(r=-0.106,P<0.05),sperm forward motility rate(r=-0.33,P<0.01),sperm normal morphology rate(r=-0.187,P<0.05),and MMP(r=-0.283,P<0.01).ROS in spermatozoa was positively correlated with DFI(r=0.225,P<0.01).However,there was no significant correlation between ROS in spermatozoa and semen volume(r=0.143,P>0.05).Conclusion:(1)The harmful components in tobacco smoke may lead to the increase of male sperm ROS through abnormal oxidative stress(OS),and then lead to the decrease of sperm concentration,motility,normal morphology rate and MMP.Besides,it could increase the sperm DFI.Finally,the male fertility declines with the increase of smoking level.(2)The sperm ROS not only reflects sperm OS level,but also reflects sperm routine parameters,sperm MMP and DFI,which suggests it can be used as one of the important indexes to predict male fertility.
