Objective To compare the clinical efficacy and survival of laparoscopy and laparotomy in the early endometrial cancer. Methods 1.Data sources: The clinical and postoperative follow-up data of 278 patients with early endometrial cancer 2021 from January 2011 to August 2011 were collected.171 patients with laparoscopy were selected as study group,107 cases of open abdominal surgery as a control group.Data were collected using an electronic medical record system and and direct telephone.2.Analysis indicators: the general information and perioperative data, intraoperative and postoperative complications were compared between the two groups,the3-year and 5-year survival rate,disease-free survival rate, total survival rate, total disease-free survival rate, recurrence rate and mortality rate were compared.3.Data collected were analyzed using SPSS 27.0 statistical software. Results 1.The amount of blood loss during operation in the laparoscopy group was significantly less than that in the laparotomy group, and the number of blood transfusions was significantly less than that in the laparotomy group(P <0.005).The length of hospital stay in the laparoscopy group was significantly shorter than that in the laparotomy group(P <001).The mean retention time of drainage tube in the laparoscopy group was significantly shorter than that in the laparotomy group. The average operation time, the number of lymph nodes removed during operation, the average recovery time of anal exsufflation, the average retention time of urinary catheter and the use of antibiotics after operation were compared between the two groups, the difference was not statistically significant.2.There was no significant difference in the incidence of intraoperative complications between the laparoscopy group and the laparotomy group, and the laparoscopy group had a lower incidence of postoperative complications and total complications than the laparotomy group, the difference was statistically significant(p <0.05).3.There was no significant difference in the recurrence rate, mortality rate,disease-free survival rate, total disease-free survival rate and total survival rate at 3 and 5 years after operation between the two groups. Conclusion 1.Laparoscopy is superior to laparotomy in the treatment of early endometrial cancer blood loss, postoperative hospital stay, and postoperative complications.2.The recurrence rate, mortality rate,3-and 5-year survival rates, and disease-free survival, overall survival, and overall disease-free survival rates were similar in the two groups. Therefore, for early endometrial cancer, the laparoscopy is as safe as the laparotomy. |
