Objective The differences in serological indicators between patients with rheumatoid arthritis(RA)in different disease activity are analyzed retrospectively to provide clues for the clinical evaluation of RA patients.Methods Collect 293 patients who were clearly diagnosed as RA and did not have other autoimmune diseases in the rheumatic immunology department of the General Hospital of Ningxia Medical University from 2018 to 2022.The activity status of the disease was calculated using the DAS28 score.According to the calculation results,it was divided into four groups.The general information,laboratory indicators,imaging examination of the selected cases were reviewed and recorded in detail,and statistical analysis.The differences between the main serological indicators between the four groups were compared,and the correlation between serological indicators and RA disease activity was analyzed.Results1.Compared with RA patients in the other three groups,the RA high disease activity group had a lower BMI(P<0.05);there was no significant statistical difference in sex,admission age,age of onset and course of disease between the four groups(P>0.05).2.The proportion of proximal interfingerial,metacarpal and wrist joints in RA high disease activity group was higher than that in the other three groups(P<0.05).The elbow joint participation rate of the active group with high-incurrence RA disease was low(P<0.05).The comparison of knee joint participants is statistically significant between 2 trillion(P>0.05).The difference in the participation of these four pairs of shoulder joints is not statistically significant(P>0.05).3.The proportion of X-ray staging in RA patients is more common in stage I,and the difference is statistically significant(P<0.05);the two comparisons between the four groups of X-ray staging groups are not statistically significant(P>0.05).4.The HGB of patients with high-disease activity group of RA is smaller than the other three groups(P<0.05);MCV and MCH in the RA high-disease activity group are smaller than that of the disease relief group and the low-disease activity group(P<0.05);the low-disease activity group of RA patients and the middle disease activity group are similar,and the remaining groups are higher than the group with low activity group(P<0.05);the PLT of patients with high disease activity group PLT in the RA high-disease activity group is greater than the other three groups(P<0.05);the PCT of the RA high-disease activity group is greater than that of the disease relief group and the low-disease activity group(P<0.05);There is no statistical difference in the remaining routine blood indicators(P>0.05).5.The FIB of patients with RA high disease activity group is higher than that in the remission group and the moderate disease activity group(P<0.05);the RA condition relief group TT was longer than the other three groups(P<0.05);D-dimer is higher in the disease activity group(P<0.05);there is no significant statistical difference in the remaining coagulation indicators(P>0.05).6.ESR in the RA high-disease activity group was faster than the other three groups(P<0.05);IL-6 was higher in the high disease activity group(P<0.05);Ig M was lower than in the moderate and high-disease activity group(P<0.05);the AKA positive rate was higher in the high disease activity group(P<0.05);there is no statistical difference in the remaining immunization indicators(P>0.05).7.The ALB and A/G of patients in the RA high disease activity group were lower than the remaining three groups(P<0.05);the GLB of the remaining three groups of RA was lower than that of the high disease activity group(P<0.05);the TG of the RA disease remission group and the low disease activity group was lower than that of the high disease activity group(P<0.05);and the differences in the remaining biochemical indicators in each group were not statistically significant(P>0.05).8.The NLR and PLR of the other three RA groups were lower than P<0.05 for patients in the high-disease activity group;HPR,ALB-d NLR and FAR in the high-disease activity group were lower than those in the other three groups(P<0.05);the CAR of patients with RA high disease activity group is higher than that in the disease remission group and the middle disease activity group(P<0.05);the remaining derivative indicators of each group did not differ statistically significantly(P>0.05).9.The difference between the four groups of patients with pulmonary interstitial disease and left ventricular diastolic dysfunction has no statistical significance(P>0.05).10.Analysis of the ROC curve of each group of serological indicators,the area of HGB,MCV,MCH,PLT,PCT,FIB,D-dimer,GLB,A/G,NLR,PLLR,HPR,ALB-d NLR,CAR and AFR curves are all above 0.6(P<0.05),the area under the AFR curve is greater than 0.7,and the application value is high;HGB,MCV,MCH,FIB,GLB,A/G,HPR,ALB-d NLR,AFR sensitivity is good,PLT,PCT,D-dimer,NLR,CAR index specificity It is better.11.For correlation analysis with DAS28,PLT,PCT,FIB,D-dimer,GLB,NLR,PLR,CAR are significantly positively correlated,and HGB,MCV,MCH,A/G,HPR,ALB-d NLR and AFR are significantly negative(P<0.05).Conclusion1.The body consumption of RA patients in the high-disease activity group is obvious,and the BMI is lower.2.The joint involvement of RA patients with high degree of disease activity is mainly the proximal interfinger joint,palm and wrist joint.3.These may provide some clues for the disease activity of RA patients which are HGB、MCV、MCH、PLT、PCT、FIB、D-dimer、GLB、A/G、NLR、PLR、HPR、ALB-d NLR、CAR、AFR.
