Effect Of Plasma Albumin Level On TCM Syndromes And Disease Activity Of Rheumatoid Arthritis

Objective:By studying the relationship between albumin level and TCM syndrome type,TCM syndrome score,DAS28 score,ESR,CRP,GLO,HGB,PLT and so on in patients with rheumatoid arthritis,it provides a reference for the identification of RA syndrome and disease activity.Methods:166 patients with RA who met the criteria were divided into six syndromes.The syndrome types of rheumatism obstruction syndrome,cold dampness blockage syndrome,dampness-heat blockage syndrome and phlegm stasis syndrome were classified as solid arthralgia,and syndrome of deficiency of both qi and blood and liver and kidney deficiency syndrome were classified as deficiency arthralgia.80 healthy subjects were used as the control group,and observation data were recorded.Statistical analysis was performed using SPSS 20.0 software.Results:1.The HGB,ALB,BMI,TG,TC,LDL-C,and UA in the RA group were lower than the control group,and the GLO and PLT were higher than the control group,and there were statistical differences(P<0.05).2.Of the 166 RA patients,53 had hypoproteinemia,accounting for 31.93%.The age,DAS28 score,CRP,ESR,RF,PLT,GLO,TCM syndrome scores of the hypoproteinemia group were higher than those of the non-hypoproteinemia group,and the HGB was lower than the non-hypoproteinemia group.There were statistical differences(P<0.05).There were no statistical differences in gender,course of disease,BMI,blood lipids and uric acid between the hypoproteinemia group and the non-hypoproteinemia group(P>0.05).3.Comparison of protein metabolism and disease activity in patients with RA of different Syndromes: The ALB of rheumatism obstruction syndrome was higher than that of other syndrome types(P<0.05).The scores of TCM syndromes of cold dampness blockage syndrome were higher than other syndrome types,and there were statistical differences(P<0.05).The DAS28 score of cold dampness blockage syndrome was higher than phlegm stasis syndrome,liver and kidney deficiency syndrome,syndrome of deficiency of both qi and blood,rheumatism obstruction syndrome(P<0.05).The DAS28 score of dampness-heat blockage syndrome was higher than phlegm stasis syndrome,syndrome of deficiency of both qi and blood,and rheumatism obstruction syndrome(P<0.05);The DAS28 score of phlegm stasis syndrome was higher than liver and kidney deficiency syndrome and rheumatism obstruction syndrome(P<0.05).The DAS28 score of liver and kidney deficiency syndrome was higher than syndrome of deficiency of both qi and blood and rheumatism obstruction syndrome(P<0.05).There were no statistical differences in HGB,GLO,PLT,RF,anti-CCP and ESR of each syndrome type(P>0.05).4.The DAS28 score,CRP,TCM syndrome scores in the group of solid arthralgia was higher than that of the group of deficiency arthralgia(P<0.05).There was no statistical difference in HGB,ALB,GLO between the two groups(P>0.05).In the solid arthralgia group,the HGB of hypoproteinemia group is lower than that of non-hypoproteinemia group,and GLO,DAS28 score,CRP,ESR,PLT,TCM syndrome scores are higher than that of non-hypoproteinemia group(P<0.05).5.In RA patients,ALB was negatively correlated with PLT,RF,CRP,ESR,TCM syndrome scores and DAS28 score(P<0.05);HGB was significantly negatively correlated with PLT,CRP,ESR and TCM syndrome scores(P<0.01);GLO was positively correlated with PLT,RF,CRP,ESR,DAS28 score and TCM syndrome scores(P<0.05).Conclusions:1.Hypoproteinemia is a common complication of RA.The patients with hypoproteinemia are older,the globulin and hemoglobin metabolism disorders are more obvious,and the disease activity is higher.2.The activity disease of solid arthralgia group was higher than that of deficiency arthralgia group;in the solid arthralgia group,the RA patients with hypoproteinemia had higher disease activity.3.Among the syndrome types of RA,the TCM syndrome scores of cold dampness blockage syndrome were the highest,the ALB of rheumatism obstruction syndrome was the highest,and the DAS28 score of rheumatism obstruction syndrome was lower than other solid arthralgia types and liver and kidney deficiency syndrome.4.The levels of ALB,HGB,TG,TC,LDL-C and UA in RA patients were lower than those in healthy people,and the GLO level was higher than that in healthy people.The activity of RA increased with the decrease of ALB and the increase of GLO,and ALB,GLO can be used as a supplementary index to judge the activity of RA disease.
