Objective Through retrospective study of patients undergoing gasless endoscopic thyroid surgery in our hospital from 2021 to 2022,a case-control study was conducted on patients with two different surgical approaches to compare the operation time,anesthesia time,drainage,hospitalization expenses,postoperative complications,postoperative pain degree and cosmetic satisfaction of the two surgical approaches.The advantages of different surgical approaches and their effects on patients are more accurate.Grasp the indications of different operations in order to better achieve the expected goals of patients and improve the postoperative aesthetics of patients.Methods From May 1,2021 to October 31,2022,64 patients who underwent gasless endoscopic thyroid surgery were collected,including 29 cases through axillary approach and 35 cases through oral vestibular approach.The perioperative conditions and follow-up data of the two groups were compared and summarized statistically by SPSS27.0.Results A total of 64 patients undergoing gasless endoscopic thyroid surgery were included according to the inclusion and exclusion established.There was no significant difference in sex,age,pathologic type,lesion diameter,surgical resection range between the two groups(P>0.05)in 29 cases of axillary approach and 35 cases of oral vestibular approach.Compared with oral vestibular approach,axillary approach has shorter operation time,shorter anesthesia time and less hospitalization cost(P<0.05);There was no significant difference between the two groups in the drainage volume 24 hours after operation and the hospitalization time after operation(P>0.05);Postoperative complications,such as skin sensation,hoarseness,drinking water choking cough,numbness of hands and feet,subcutaneous effusion,and postoperative incision infection,had no significant difference(P>0.05);There was no significant difference in postoperative pain score and cosmetic satisfaction between the two groups.Conclusion The operation time and learning curve of the axillary approach are superior to those of the oral vestibular approach,and are more suitable for beginners in endoscopic thyroid surgery.Both surgical approaches are safe and feasible.The hospitalization cost of the axillary approach is lower than that of the oral vestibular approach,and patients can choose a suitable surgical method according to their own cosmetic needs and economic capabilities.
