A Clinical Comparative Study Between Endoscopic Thyroidectomy And Traditional Open Thyroidectomy In The Treatment Of Differentiated Thyroid Carcinoma

Objective: To compare the clinical efficacy of endoscopic thyroidectomy(ET)and traditional open thyroidectomy(OT)in the treatment of differentiated thyroid carcinoma(DTC).Methods: By reviewing and analyzing,27 cases of patients with differentiated thyroid carcinoma underwent total endoscopic thyroidectomy in the Department of General Surgery were selected as endoscopic thyroidectomy group,while 30 cases underwent traditional open thyroidectomy were selected as open thyroidectomy group,ranging from January 2014 to September 2016 in The First Affiliated Hospital of Guangxi Medical University.The operative time,intraoperative blood loss,total drainage amount after operation,length of hospital stay after operation,number to times analgesic was used after operation,the number of central lymph node dissected,post-operative complication rate and tumor recurrence rate were compared.Results: All patients underwent a successful operation.There were no serious postoperative complications.There were no cases of endoscopic thyroidectomy operation that needed to be converted into open thyroidectomy.Compared to the OT group,the operative time and the length of hospital stay after operation in the ET group was longer and the total drainage amount after operation was more,but the intraoperative blood loss was less,and the difference between the two groups were statistically significant(P< 0.05).There were no significant difference between the two groups in the number of central lymph node dissected,number to times analgesic was used after operation,post-operative complication rate and tumor recurrence rate(P>0.05).Conclusions:1.As for the diameter?4cm,with no lymph node metastasis of the early-stage differentiated thyroid carcinoma,endoscopic thyroidectomy is safe and feasible,the number of central lymph node dissected can achieve the same effect as in traditional open thyroidectomy,but its long-term effect still require further follow-up study.2.Endoscopic thyroidectomy has the advantages of less intraoperative blood loss,smaller incision and better cosmetic effect,while the disadvantages are longer operative time,longer length of hospital stay after operation,increase in the total amount of drainage after operation,difficult to operate,and the application of cases with extensive cervical lymph node metastasis are limited.
