With the development of economy in our country,more and more people pay attention to mental health and mental disorders.Resistance to treatment is a phenomenon that distinguishes mental disorders from most other diseases.The lack of insight of patients with mental and psychological disorders to their own diseases means that patients will deny their own mental and psychological abnormal states or symptoms,which often indicates that patients are difficult to maintain relevant treatment,which is not conducive to the long-term stability of patients with symptoms and social rehabilitation.In clinical work,improving the treatment compliance of patients with mental disorders is the key to the treatment process.The lack of insight of patients will make it difficult to establish a good doctor-patient treatment alliance,making it difficult to carry out and continue all treatments.Impairments in insight can manifest themselves in a variety of forms,both in clinical Settings and in the daily lives of people with mental disorders.Insight disorder is the result of a variety of factors,and is closely related to the patient’s symptoms,personality characteristics and social and cultural characteristics.With the development of modernism to post-modernism,contemporary people have new attitudes and views on mental disorders,and the same is phenomenon of insight.This paper adopts the research paradigm of qualitative research and the research method of grounded theory.Taking the inpatient ward of mental disorders as the research place,the patients were regarded as the main body,the clinical work role and patients were interviewed,and the formation and operation mechanism of patients’ insight phenomenon were explored.The study was conducted in three parts.Research theme 1,it analyzed and explored the reaction of patients with mental and psychological disorders who were believed to have mental and psychological disorders,the explanation of the disease and the relevant mechanism of action.Research theme 2,it analyzed and explored the reaction,formation of patients with mental and psychological disorders who resisted treatment and the relevant mechanism of action.Research theme 3,Explore the formation and mechanism of insight.The differences of different roles on the same topic in clinical work scenarios were analyzed.At last,it further analyzes and summarizes from the perspective of subjectivity and intersubjectivity,and puts forward new opinions and views.Results: Study 1.When the patient is considered by others to have a mental disorder or "seizure" of symptoms,his mood is in a state of high tension,and his behavioral response includes two behavioral directions,inward behavior and outward behavior,mainly the outward behavior response.The explanation of disease includes two perspectives: focusing on the symptoms and focusing on the background.Study 2.The reasons for patients to resist treatment include four aspects: symptom state,relationship characteristics,personality characteristics and treatment process characteristics.The forms of resistance treatment include direct resistance and conversion resistance,and conversion resistance is the main form of resistance.The patient’s perception of treatment includes three parts: the will of others,the irrelevance of disease and the impairment of efficacy.Study 3.The reasons that patients are not aware of include trait defect and state impairment.The reasons of denial included four aspects: internalized stigma,adverse reality,negative experience and integration difficulties.The population was characterized by patients with hope,interpersonal difficulties and related unfamiliar.The reasons of patients’ insufficient knowledge include information channel,personal defect and avoidance attitude.Patients’ perception of disease included low self-efficacy and high negative gaze.Conclusion: The stigma of patients with mental disorders consists of two independent psychological dimensions: negative gaze and self-efficacy.Other characters’ excessive attention to the disease will strengthen the patient’s gaze and lead to high tension interaction;The negative experience of patients participating in treatment will damage patients’ sense of efficacy of treatment.The above parts form the patient’s insight under the comprehensive effect of the recognition function and the integration function of the patient’s subject.
