Analysis Of The Effect Of The Establishment Of Trauma Center On The Treatment Of Severe Trauma Complicated With Fracture

Objective:This study analyzed the epidemiological characteristics of trauma patients,and the treatment effect for severe trauma patients with fracture before and after the establishment of trauma center,aiming to explore a trauma treatment model suitable for our region,improve the construction of trauma center and improve the ability of trauma treatment.Method:The trauma patients admitted to Guilin People’s Hospital from May 2018 to April 2022 were retrospectively analyzed.According to the inclusion criteria and exclusion criteria,117 patients who met the research criteria from May 2018 to December 2019 were taken as the control group,and162 patients who met the research criteria from January 2020 to April 2022 were taken as the research group.The differences in pre-hospital time,emergency treatment time,time to complete basic imaging examination,mortality rate and length of hospital stay etc between the study group and the control group were compared.P<0.05 is statistically significant.Result:There was no statistical difference in sex,age,ISS score and injury factors between the two groups.There are more men than women in trauma patients.Traffic injuries,falls and falling from height are the top three causes of injuries.Trauma leads to limb injuries and fractures in the largest proportion,followed by patients with craniocerebral injury and skull fracture.Compared with the control group,in the study group,The proportion of pre-hospital time less than 2 hours has increased from 46.1% to 50.0%,and the proportion of more than 4 hours has increased from 24.8% to 33.3%.The emergency room time was shortened from 70.1±7.8 minutes to 50.7±6.3 minutes.The time to complete the basic image examination was shortened from 28.8±5.2 minutes to23.3±4.1 minutes.The total length of hospitalization was shortened from 26.8±4.4 days to 22.6±5.8 days.The mortality rate in ICU decreased from 55.2%to 19.7%,the mortality rate of serious complications decreased from 45.5% to32.7%,and the total mortality rate decreased from 15.4% to 7.4%.There was no significant difference in emergency operation rate,operation mortality rate,length of stay in ICU and duration of ventilator use,blood transfusion rate and mortality rate of pelvic fracture between the two groups.Conclusion: After the establishment of the trauma center,the emergency room treatment time,the time to complete the basic imaging examination and the total hospitalization time were shortened,and the mortality rate of severe trauma patients with fractures was effectively reduced.To some extent,the trauma center has played a positive role.
