Abstract/Summary:
BackgroundIntracranial aneurysms(IA)are caused by a localized,pathologically dilated intracranial artery wall and are at risk of rupture,and rupture of aneurysms is the first cause of spontaneous subarachnoid hemorrhage(SAH)[1-1].The consequences of aneurysmal SAH caused by sudden rupture of intracranial aneurysms are serious,including a large number of cases,a wide range involved,and poor prognosis,which is a cerebrovascular disease that seriously endangers human health[2],and is one of the common diseases in neurosurgery.Early identification of patients with intracranial aneurysms who are at high risk of rupture but have not yet ruptured through certain simple tests is critical.PourposeThe correlation between ruptured intracranial aneurysm hemorrhage and apolipoprotein A1(Apolipoprotein A1,Apo-A1),apolipoprotein B(Apolipoprotein-B,Apo-B)and Apo-A1/B was discussed,and the risk of intracranial aneurysm rupture was predicted by the values of Apo-A1,Apo-B and Apo-A1/B,which provided a theoretical basis for clinical prevention and treatment of intracranial aneurysm.MethodsA retrospective analysis of the clinical relevant data of 130 patients with intracranial aneurysms diagnosed in the Department of Neurosurgery of the Third Affiliated Hospital of Guangzhou Medical University from January 2017 to December 2022 divided the intracranial aneurysms into two groups,one was the rupture group and the other group was the unruptured group,of which 26 cases were in the ruptured group,accounting for 20%,and 104 cases were in the unruptured group,accounting for 80%.General clinical data include gender,age,history of hypertension,diabetes,alcohol consumption,smoking,diameter of intracranial aneurysm,location of intracranial aneurysm,width of intracranial aneurysm neck,total cholesterol(TC),triglyceride(TG),Apo-A1,Apo-B and Apo-A1/B,high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),lipoprotein(a)(LP(a),apolipoprotein E(Apo E),fasting blood glucose level.Firstly,these influencing factors of aneurysm patients in the ruptured group and the non-ruptured group were analyzed univariately,and the difference was not statistically significant with P>0.05 to exclude the influencing factors,and then the risk factors with statistical significance were screened out by P<0.05,and then the independent risk factors for ruptured intracranial aneurysm bleeding were screened out by multivariate logistic regression analysis.ResultsUnivariate analysis showed that the ruptured and unruptured intracranial aneurysm group had aneurysm location(P=0.002),aneurysm morphology(P=0.004),tumor neck width(P=0.049),Apo-A1(P<0.001),Apo-B(P<0.001),Apo-A1/B(P The P value on<0.001)and fasting blood glucose(P=0.024)was less than 0.05,and the difference was statistically significant,indicating that the rupture and bleeding of intracranial aneurysm were correlated with the location of the aneurysm,aneurysm morphology,tumor neck width and narrowness,Apo-A1,Apo-B,Apo-A1/B and fasting blood glucose values.Multivariate logistic regression analysis showed that the P value of intracranial aneurysm morphology,tumor neck width,Apo-A1/B and fasting blood glucose were greater than 0.05,and the difference was not statistically significant,that is,it was not an independent risk factor for intracranial aneurysm rupture and bleeding,while the location of the aneurysm,the P value of Apo-A1 and Apo-B were less than 0.05,the difference was statistically significant,indicating that the location of the aneurysm,Apo-A1 and Apo-B values were independent risk factors for ruptured intracranial aneurysm,and the correspondingβvalues of Apo-A1 and Apo-B were-2.502 and 1.637,respectively,that is,Apo-A1 was negatively correlated with intracranial aneurysm rupture and bleeding,and Apo-B was positively correlated with intracranial aneurysm rupture bleeding.ConclusionIntracranial aneurysm rupture and bleeding were associated with Apo-A1,Apo-B,and Apo-A1/B values,which were independent risk factors for intracranial aneurysm rupture and bleeding,and Apo-A1 was negatively correlated with intracranial aneurysm rupture and bleeding,and Apo-B was positively correlated with intracranial aneurysm rupture and bleeding.
