| Background: Herpes zoster(HZ)is a common viral infectious skin disease caused by varicella-zoster virus(VZV).The new version of expert consensus on the diagnosis and treatment of herpes zoster in China [1] recommends the use of acyclovir antiviral therapy,but the use of adenosine monophosphate for antiviral therapy is also common in clinical practice.Therefore,this study provides more options for the clinical treatment of HZ by exploring the different efficacy of two antiviral drugs.Objective: To compare the efficacy of adenosine monophosphate and acyclovir in the treatment of HZ.Methods: In this study,a total of 40 HZ hospitalized patients were selected from the Fifth People’s Hospital of Chengdu from April 2022 to September2022.40 patients were randomly divided into 2 groups,with 20 patients in each group.Group A received acyclovir 7mg/kg q8 h,group B received adenosine monophosphate 7mg/kg qd,as well as neurotrophic,analgesic and physical therapy.After 7 days of treatment,we compared(1)VZV DNA concentration,(2)revised Wong‐Baker Facial expression Pain Assessment(FPS-R)score,(3)time to pain relief,time to blister,time to scab,time to scab shedding,(4)comparison of overall efficacy,and(5)whether there was residual neuropathy.After SPSS26.0 statistical analysis,the antiviral efficacy of the two was compared.Results: 40 patients with herpes zoster were included in this study,20 in group A and 20 in group B.There was no statistical difference in baseline data and the two groups were comparable.(1)VZV DNA concentration: Group A was11.090±2.420ug/ml before treatment,and group B was 10.520±2.289 ug/ml before treatment.There was no statistical significance in VZV DNA concentration between the two groups(P =0.595>0.05).After treatment,group A was 5.000±1.059ug/ml and group B was 8.210±2.382ug/ml,both of which were statistically significant(P =0.001<0.05).The concentration of VZV DNA in group A decreased more obviously.(2)FPS-R pain score: 5.500±1.395 before treatment in group A and 6.200±1.281 before treatment in group B,P =0.107>0.05,the difference was not statistically significant;After treatment,group A was 1.600±1.273 and group B was 2.500±1.147,P =0.024< 0.05.The pain scores of both groups were significantly decreased,and the decrease was more obvious in group A,the difference was statistically significant.(3)Symptom improvement: In terms of pain relief time,there were 2.600±1.429 days in group A and 3.950±1.701 days in group B,both of which had statistical significance,while there were no statistical significance in blister time,scab time and scab shedding time.(4)Comparison of efficacy: On page 3,the effective rates of group A and Group B were 85% and 70%,respectively.The U test showed that P > 0.05,showing no statistical significance.(5)Residual neuralgia: There were 4 patients in group A(20%)and 6 patients in group B(30%)with residual neuralgia.Chi-square test showed that there was no statistical significance between the two groups,with P > 0.05.Conclusion: Acyclovir has stronger antiviral effect and faster pain relief in the treatment of HZ.The results of other curative measures were similar.Therefore,acyclovir is still the best choice for the clinical treatment of HZ. |
