Abstract/Summary:
Objective: We used voxel-based morphometry(VBM)and regional homogeneity(ReHo)methods to evaluate the morphological and functional changes of essential hypertension patients with sleep disorders. To study the brain morphological and functional imaging manifestations in patients with essential hypertension accompanied by sleep disorders,in order to discover the neuroimaging mechanism related to hypertension accompanied by sleep disorders,so as to provide help for clinical research on brain structural and functional damage of essential hypertension accompanied by sleep disorders. Methods:Patients with clinical diagnosis of hypertension were selected from the Affiliated Hospital of Gansu University of Traditional Chinese Medicine from January to September2022.All participants in the experiment were right-handed,with a total of 33 cases.Among them, there were 26 patients with hypertension and sleep disorder, which were HTN and sleep disorder(SD)group. There were 7 cases without sleep disorder, which were HTN group alone. In addition,22 healthy people with the same age and sex were selected as healthy control group(HC).The three groups underwent routine MRI scanning,3D-T1 sequence and FE-EPI sequence scanning. The data of 3D-T1 sequence and FE-EPI sequence of magnetic resonance images of all subjects were preprocessed by VBM8 of SPM software on MATLAB platform, and the processed brain map was obtained. Then the gray matter volume map and Reho value of the three groups of subjects were calculated, and the gray matter volume map and Reho value of the three groups of subjects were statistically analyzed. The imaging changes of patients with hypertension and sleep disorder and the differences between patients with general hypertension and healthy people were analyzed. The statistical analysis function of SPSS 22.0 software was used to analyze the age and gender of all subjects.Spearman correlation analysis was performed for hypertension duration and Pittsburgh Sleep Quality Index(PSQI)score.Results:1.Compared with HC group, there was less gray matter in the left precuneus of HTN combined with SD group.Compared with HTN alone, the volume of gray matter in the left precuneus of HTN combined with SD was reduced. There was no significant difference in gray matter volume between HC group and HTN group.2.Compared with HC,the ReHo value of the right superior margin gyrus decreased in HTN combined with SD group.Compared with HC,the ReHo value of HTN decreased. There was no significant difference in the ReHo value between HTN combined with SD and HTN alone.3.The duration of hypertension was positively correlated with Pittsburgh Sleep Quality Index(PSQI)score(R=0.544,P<0.05).Conclusion:1.The decrease of gray matter volume and ReHo value in the precuneus and superior margin gyrus in the HTN combined with SD group indicates that the precuneus and superior margin gyrus may be involved in the occurrence and development of hypertension and sleep disorders. The decrease of gray matter volume in the precuneus of HTN patients combined with SD group may indicate that the precuneus is related to sleep disorders.2.VBM and ReHo were able to detect differences in brain morphology and function between patients with and without sleep disorders in the diagnosis of hypertension and sleep disorders.3.The hypertension course of patients in HTN combined with SD group was positively correlated with the score of Pittsburgh Sleep Quality Index.
