Objective:To observe the effect of Gushuyanjin exercise on the improvement of thirst sensation in patients with heart failure of deficiency of both Qi and Yin,this study finds out the effective intervention method for thirst in patients with heart failure,provides a reference for thirst nursing in patients with heart failure,and also accumulates experience for the application of traditional Chinese medicine health exercise in clinical nursing work.Methods : According to the inclusion and exclusion criteria,this study selected 70 patients with heart failure from January 2022 to October 2022 in a tertiary hospital in Gansu Province as the study subjects,during the intervention,the control group fell off 1 case,the intervention group shed 1 case,and eliminated 1 case,and finally the total sample size was 67 cases,including 34 cases in the control group and 33 cases in the intervention group,the control group was given routine care,and the intervention group was given Gushuyanjin exercise on the basis of the control group,and the patients were collected before intervention,Visual Analogue Scale(VAS values),Thirst Distress Scal-Heart Failure score(TDS-HF score),thirst duration time,and Unstimulated Whole Saliva(UWS)on day 1,day 5 and day 10 of the intervention.SPSS 26.0 software is used for statistical analysis,two independent samples t-test is used for the measurement data that obeyed the normal distribution,and the rank sum test is used for the measurement data and grade data that do not follow the normal distribution,and the difference is statistically significant with P<0.05.In order to exclude the effects of time and confounding factors,multivariate repeated measurement ANOVA is used to determine the effect of the intervention.Results:(1)Comparison of general data between the two groups: there were no statistically significant differences in demographic data(gender,age,marital status,residence,education level)and disease-related data(cardiac function classification,primary disease,medication,TCM syndrome score,course of disease,BMI,NT-Pro BNP,LVEF)between the two groups(P>0.05).There are no statistically significant differences in VAS value,distribution of thirst degree,TDS-HF score,thirst duration score and UWS between the two groups before intervention(P>0.05),which are also comparable.(2)The VAS values of the two groups are compared: The results of repeated measures analysis of variance show that the time effect,group effect and interaction of VAS values in the two groups are statistically significant(P<0.05).There is no statistically significant difference in VAS value between the two groups at baseline(T0)and the first day of intervention(T1)(P>0.05),but there is statistically significant difference in VAS value between the two groups on the fifth day of intervention(T2)and the tenth day of intervention(T3)(P<0.05).For intra-group comparison,the single effect of time factor on VAS in the intervention group is statistically significant(P<0.05).There are statistically significant differences in VAS values between baseline(T0)and the first day(T1),the fifth day(T2)and the tenth day(T3)of intervention in the intervention group(P<0.05).(3)Comparison of the distribution of thirst degree between the two groups: On the first day of intervention(T1),there is no statistically significant difference in the distribution of thirst degree between the two groups(P>0.05).On the 5th day(T2)and 10 th day(T3)of intervention,the difference in the distribution of thirst degree between the two groups is statistically significant(P<0.05).(4)The TDS-HF scores of the two groups are compared : Comparison of the total TDS-HF score between the two groups: Repeated measures analysis of variance show that the time effect,group effect and interaction of the total TDS-HF score of the two groups are statistically significant(P<0.05).There is no significant difference in the total score of TDS-HF between the two groups at baseline(T0)and on the first day of intervention(T1)(P>0.05).There are statistically significant differences in the total score of TDS-HF between the two groups on the 5th day(T2)and 10 th day(T3)of intervention(P<0.05).For intra-group comparison,the single effect of time factor on the total score of TDS-HF in the intervention group is statistically significant(P<0.05).There is no significant difference in the total score of TDS-HF between baseline(T0)and the first day of intervention(T1)in the intervention group(P>0.05),but there is significant difference in the total score of TDS-HF between the fifth day(T2)and the tenth day(T3)in the intervention group(P<0.05).Comparison of the item scores of TDS-HF between the two groups: there is no statistically significant difference in the item scores of TDS-HF between the two groups before intervention(P>0.05).The item scores of TDS-HF in the intervention group on the 10 th day of intervention are significantly different from those before intervention(P<0.05).(5)Comparison of thirst duration between the two groups: The results of repeated measures analysis of variance show that the time effect,group effect and interaction of the thirst duration score of the two groups are statistically significant(P<0.05).There is no significant difference in the score of thirst duration between the two groups at baseline(T0)and on the first day of intervention(T1)(P>0.05).On the 5th day(T2)and 10 th day(T3)of intervention,there are statistically significant differences in the scores of thirst duration between the two groups(P<0.05).For intra-group comparison,the single effect of time factor on thirst duration score in the intervention group is statistically significant(P<0.05).There is no significant difference in the score of thirst duration between baseline(T0)and the first day(T1)of intervention in the intervention group(P>0.05),but there is significant difference in the score of thirst duration between the fifth day(T2)and the tenth day(T3)of intervention in the intervention group(P<0.05).(6)The UWS of the two groups are compared:The results of repeated measures analysis of variance show that the time effect,group effect and interaction of UWS in the two groups are statistically significant(P<0.05).There is no significant difference in the score of thirst duration between the two groups at baseline(T0)and on the first day of intervention(T1)(P>0.05).On the 5th day(T2)and 10 th day(T3)of intervention,there are statistically significant differences in the scores of thirst duration between the two groups(P<0.05).For intra-group comparison,the single effect of time factor on UWS in the intervention group is statistically significant(P<0.05).There are significant differences in UWS between baseline(T0)and the first day(T1),fifth day(T2)and tenth day(T3)of intervention in the intervention group(P<0.05).Conclusion:(1)This study use Gushuyanjin exercise to intervene thirsty patients with heart failure of Qi and Yin deficiency.The results show that compared with routine nursing,Gushuyanjin exercise could effectively reduce the patients’ thirst intensity,alleviate the degree of thirst distress,shorten the time of thirst duration,and increase the UWS,so as to improve the patients’ thirst symptoms.(2)The intervention method of Gushuyanjin exercise has significant effect and good clinical application value.
