Objective:Individualized nutritional intervention was carried out for patients with liver cirrhosis,and its impact on the nutritional status and quality of life of patients was analyzed,It aims to improve the nutritional status of patients and improve their quality of life.Methods:86 patients with liver cirrhosis who met the inclusion criteria and volunteered to participate in this study who were admitted to the Department of Gastroenterology of a Class III hospital in Shanxi Province from April to October 2022 were selected as the study subjects.The subjects were randomly divided into control group(n=43)and intervention group(n=43)by random number table.Control group:patients received general comprehensive treatment and routine nutrition intervention,and were treated according to the routine of department diagnosis,treatment and nursing.Intervention group:based on the control group,the nutrition intervention group developed and implemented individualized nutrition intervention plan for patients.In this study,patients were evaluated and followed up three times within 48 hours after admission,1 month after discharge and 2months after discharge.General data of patients were collected and statistically analyzed before and after intervention.NRS2002,subjective global assessment(SGA),hemoglobin(HB),serum albumin(ALB),prothrombin time(PT),body mass index(BMI),midarm muscle circumference(MAMC)Handgrip strength(HG),health survey summary(SF-36),Child-pugh grade and new complications(including gastrointestinal bleeding,hepatic encephalopathy,hepatorenal syndrome,ascites,infection and other symptoms)were statistically analyzed,and the improvement of nutritional indicators and quality of life indicators of the two groups were compared.SPSS 26.0 software was used for statistical analysis of the data,and the measurement data were described with mean±standard deviation.After normal distribution and variance homogeneity test,independent sample t-test was used for comparison between the two groups;The counting data is statistically described by frequency and percentage(%)2Inspectionα=0.05,P<0.05,the difference was statistically significant.Results:1.Before intervention:1.1There was no statistically significant difference between the control group and the intervention group in terms of gender,age,marital status,education level,family monthly income per capita,number of hospitalization,length of illness,etc.(P>0.05),which was comparable;1.2 There were no statistically significant differences in the nutritional indicators(hemoglobin,serum albumin,prothrombin time),body mass index,upper arm muscle circumference and grip strength scores between the control group and the intervention group(P>0.05),which were comparable;1.3 There was no significant difference in NRS2002 score,SGA score,SF-36 score,original complications and Child-pugh grade between the control group and the intervention group(P>0.05),which was comparable.2.After intervention:2.1 Nutritional indicators:with the extension of intervention time,there were significant differences between the control group and the intervention group in hemoglobin,serum albumin and prothrombin time 1 month and 2 months after discharge(P<0.05);2.2 Nutritional indicators:with the extension of intervention time,there were significant differences in body mass index,upper arm muscle circumference and grip strength scores between the control group and the intervention group 1 month after discharge and 2 months after discharge(P<0.05);2.3 Nutrition assessment:2.3.1 Two months after discharge,the NRS-2002 score of the intervention group(3.08±0.53)was significantly lower than that of the control group(3.68±0.63),and the difference was statistically significant(t=4.779,P<0.05);2.3.2 Two months after discharge,the SGA scores of patients in the two groups were significantly different(~2=9.946,P<0.05);2.4 Two months after discharge,the Child-pugh grade of patients in the two groups(~2=9.982,P<0.05)was statistically significant;2.5 New complications:2 months after discharge,the incidence of new complications in the intervention group(4.65%)was significantly lower than that in the control group(13.95%),and the incidence of new complications in the two groups was statistically significant(P<0.05);2.6 Short scale score of health survey:The total score of SF-36 scale between the two groups was statistically significant(P<0.05).Conclusion:1.This study established a nutrition intervention group consisting of a physician in charge,a nutritionist and a nurse in charge to jointly develop an individualized nutrition intervention plan for patients.The group health management model ensures the scientificity and feasibility of the intervention plan,which can provide reference for clinical nursing practice.2.The intervention study on patients with liver cirrhosis has improved their nutritional status and quality of life to a certain extent,and proved the effectiveness of the program.However,due to the short intervention time,the long-term effect could not be expected,suggesting that the intervention time should be extended in the future to verify the long-term effect of the program.
